目的 观察高压氧(HBO)治疗对外伤性中、重型颅脑损伤患者血清C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)的影响及其治疗效果.方法 将108例外伤性中、重型颅脑损伤患者按完全随机设计分组法分为对照组和高压氧组,每组54例.对照组行必要的神经外科处理及常规药物治疗;高压氧组在上述治疗的基础上,在病情稳定后加用高压氧治疗1个疗程,压力0.2 Mpa(2.0 ATA),每日1次,共10次.治疗前、后分别采用免疫比浊法和ELISA法检测血清CRP、TNF-α水平,采用格拉斯哥昏迷量表(GCS)进行评分;伤后6个月随访时采用格拉斯哥预后量表(GOS)进行评估.结果 治疗前,2组患者CRP、TNF-α水平及GCS评分比较,差异均无统计学意义(P>0.05);治疗后,2组CRP、TNF-α水平均较治疗前明显降低(P<0.05),GCS评分较治疗前均明显提高(P<0.05);与对照组比较,高压氧组CRP、TNF-α水平降低及GCS评分提高更为明显(P<0.05);伤后6个月GOS评估结果显示,高压氧组预后良好患者明显多于对照组(P<0.05).结论 高压氧辅助治疗可明显降低颅脑损伤患者血清CRP、TNF-α水平,促进神经功能的恢复,提高临床疗效.
Abstract:
Objective To investigate effects of hyperbaric oxygen (HBO) therapy on C-reactive protein (CRP) and tumor necrosis factor-α (TNF-α) levels in patients with moderate or severe traumatic brain injury (TBI)and to analyze its therapeutic efficacy. Methods One hundred and eight patients with moderate or severe TBI were randomly divided into a control group (54 cases) and an HBO adjunctive therapy group (HBO group, 54 cases).Both groups received essential neurosurgical treatment and conventional drug treatment, and the HBO group was given one session of HBO therapy in addition. Serum CRP and TNF-α were detected, and the scores on the Glasgow coma scale (GCS) were measured before and after treatment. CRP was detected by turbidimetric immunoassay and TNF-α using ELISA. Glasgow outcome scale (GOS) scores were evaluated in a follow-up 6 months after injury. Results Average CRP, TNF-α and GCS measurements showed no statistically significant difference between the groups before treatment. After treatment, CRP and TNF-α were significantly lower and GCS scores significantly better in both groups, but patients in the HBO group were, on average, significantly better than the controls on all three measures.Six months later, GOS evaluation gave a significantly larger number of patients with a better prognosis in the HBO group compared with the controls. Conclusion HBO therapy can significantly decrease serum CRP and TNF-α after severe TBI, thus enhancing therapeutic efficacy.  相似文献

Objective To study the effect of hyperbaric oxygenation(HBO) therapy on the activities of serum SOD and CRP of patients with cerebral infarction and its clinical significance. Methods 78 patients with cerebral infarction were randomly divided into hyperbaric oxygen group (HBO Group, 40 cases)and clinical group (38 cases). The clinical group was treated with clinical conventional treatment only. HBO group was treated with HBO combined with clinical conventional treatment. 21 healthy volunteers were assigned to control group. Before and after treatment, the activities of serum SOD and CRP level were measured, and the curative effect were evaluated. Results The pretreatment serum SOD and CRP of the HBO group and clinical group were significantly different from the control group (P< 0. 01). Compared with the pretreatment and posttreatment serum level of the HBO group, SOD raised, CRP lowered, the difference was significant(P< 0. 01).Compared with clinical group, SOD raised, CRP lowered and the total cure rate of the HBO group were more significant different(P <0.01). Conclusion HBO therapy raised the activities of SOD and lower the level of CRP in serum of the patients with cerebral infarction, and improved the free radicals scavenging ability in the body. Therefore, HBO therapy has a good effect for cerebral infarction. The experiment suggests that the activities of SOD and CRP level in serum can be used to judge the curative effect and prognosis of HBO therapy for cerebral infarction.  相似文献

BACKGROUND:The aim of this study is to investigate the diagnostic and prognostic value of neutrophil CD64(nCD64)as a novel biomarker in sepsis patients.METHODS:One hundred fifty-one adult patients diagnosed with sepsis and 20 age-matched healthy controls were enrolled in the study.Patients with sepsis were further subdivided into a sepsis group and a septic shock group.nCD64 expression,serum procalcitonin(PCT)level,C-reactive protein(CRP)level,and white blood cell(WBC)count were obtained for each patient,and Sequential Organ Failure Assessment(SOFA)scores were calculated.RESULTS:nCD64 expression was higher in the sepsis group with confirmed infection than in the control group.The receiver operating characteristic(ROC)curve of nCD64 was higher than those of SOFA score,PCT,CRP and WBC for diagnosing infection.The area under the curve(AUC)of nCD64 combined with SOFA score was the highest for all parameters.The AUC of nCD64 for predicting 28-day mortality in sepsis was signifi cantly higher than those of PCT,CRP,and WBC,but slightly lower than that of SOFA score.The AUC of nCD64 or PCT combined with SOFA score was signifi cantly higher than that of any single parameter for predicting 28-day mortality.CONCLUSION:nCD64 expression and SOFA score are valuable parameters for early diagnosis of infection and prognostic evaluation of sepsis patients.  相似文献

Objective To explore the prognosis value of serum hydrogen sulfide (H2S) concentration and acute physiology and chronic health evaluation HI (APACHE Ⅲ) scores in critically ill patients. Methods The serum H2S levels were measured in 72 cases and 12 controls by spectrophotometry. The APACHE Ⅲ scores of the patients were assessed. The relationship between serum H2S concentration and APACHE H scores and prognosis were analyzed. Results The average serum H2S concentration of the 72 cases was (45. 6 ±17.2) μmol/L The concentration in the survival group was(41. 1 ± 14. 7) μmol/L,and which was significantly lower than in the dead group(62. 8 ±15. 5)μmol/L,(P<0. 01). The average APACHE I score was(43. 6 ±26. 0)of the 72 cases. The APACHE Ⅲ score was(37. 7 ±22. 2) in the survival group,and which was significantly lower than in the dead group (65. 0 ±29. 6) (P <0. 01). The higher of the serum H2S concentration or APACHE Ⅲ scores,the higher mortality of the patients(P <0. 01). Conclusions Serum H2S levels and APACHE Ⅲ scores of critically ill patients have a role in judging the condition and prognosis of the patients.  相似文献

Objective To determine the effects of the early enteric nutrition (EEN) up to scratch on the outcomes of the critical care patients with different degrees of severity of illness. Method There were 192 critically ill patients eligible for enrollment for study during the past 18 months in our ICU. They were classified by using APACHE Ⅱ scores. The aim of this retrospective analysis of the early enteric nutrition was to see if the enteric nufore, the patients were divided into two groups: up to scratch group and not up to scratch group. According to APACHE Ⅱ scores, the patients of each group were further divided into three sub-groups in terms of scores below 15, between15 and 25, and above 25, respectively. Results There were 62 patients in the group of EEN up to scratch, and 130 patients' EEN did not up to scratch. When the scores of APACHE Ⅱ were below 15, the length of hospital stay (LOS) was significantly shorter in group of EEN up to scratch in comparison with that of EEN not up to scratch (t = 6.453, P = 0.000). When the scores of APACHE Ⅱ were between15 and 25, the LOS in ICU (t = 3.966, P = 0.000), in hospital (t = 8.165,P = 0.000), The cost of medical care (t = 4.812,P= 0.000) and the mortality (x2 = 5.421,P = 0.038) were all significantly less in patients with EEN up to scratch. However, when the scores of APACHE Ⅱ were above 25, only the cost of medical care ( t = 7.364, P = 0.000) was significantly lower in patients of EEN up to scratch than that of EEN not up to scratch. Conclusions The EEN up to scratch can significantly improve the outcomes of critical patients and the clinical value of EEN up to scratch depends on the severity of illness.  相似文献

目的 观察心理干预联合电针及高压氧治疗脑外伤(TBI)后抑郁症的疗效.方法 共选取TBI后抑郁症患者45例,采用随机数字表法将其分为治疗组及对照组.2组患者均给予氟西汀等常规药物治疗,治疗组则在上述基础上辅以心理干预、电针及高压氧联合治疗.于治疗前、治疗8周后分别采用汉密尔顿抑郁量表(HAMD)、改良爱丁堡-斯堪的那维亚卒中量表(MESSS)及生活质量指标问卷(QL-INDEX)对2组患者抑郁程度、神经功能缺损及生活质量进行评定.结果 治疗前2组患者HAMD、MESSS及QL-INDEX评分组间差异均无统计学意义(均P>0.05);经8周治疗后,发现2组患者上述指标均较治疗前明显改善(P<0.05),且以治疗组的改善幅度相对较显著,与对照组间差异具有统计学意义(P<0.05).结论 心理干预联合电针及高压氧治疗有助于进一步改善TBI后抑郁症患者神经受损功能,减轻抑郁程度,提高生活质量,该联合疗法值得临床推广、应用.
Abstract:
Objective To investigate the effect of psychological intervention combined with electroacupuncture and hyperbaric oxygen therapy on patients with depression after traumatic brain injury (TBI). Methods Forty-five patients with depression after TBI were randomly divided into a treatment group and a control group. The two groups were both treated with routine medication and anti-depression drugs ( Fluoxetine ) for 8 weeks as a routine course of treatment, but psychological intervention combined with electroacupuncture and hyperbaric oxygen therapy was added to the treatment group's regimen. Hamilton Depression Scale ( HAMD), Modified Edingberg Scandinavian Stroke Scale (MESSS) and Quality of Life-index (QL-INDEX) scores were evaluated at the end of 8 weeks.Results There was no significant difference in the average HAMD, MESSS or QL-INDEX scores of the two groups before therapy. After treatment, the average HAMD and MESSS scores of the treatment group were significantly lower than those of the control group, and the average QL-INDEX score was significantly higher. Conclusions Psychological intervention combined with electroacupuncture and hyperbaric oxygen therapy plays an important role in the treatment of patients with depression after TBI to improve their mental status, treatment effectiveness and quality of life.  相似文献

Objective To examine the impact of C-reactive protein (CRP) on the expression of interleukin-6 (IL-6), inflammatory cytokine, in cultured human pulmonary artery smooth muscle cells (hPASMCs) in order to find out the cause of pulmonary artery hypertension (PAH). Method The hPASMCs were cultured and stimulated by different concerntrations of CRP (5 - 200 μg/ml) for different lengths of time. The activity of nuclear factor-κB (NF-κB) was evaluated by electrophoretic gel mobility shift assay (EMSA). The expression of IL-6 mRNA and the level of IL-6 protein were measured by using real-time PCR and ELISA, respectively. Results CRP increased IL-6 production in hPASMCs in a dose-dependent manner. The increase in IL-6 at concerntration of 200 μg/mL in the CRP group was as high as 2.8times that in the control group. CRP also significantly induced the activation of NF-κB in hPASMCs. The effect of CRP on the inflammatory cytokine, IL-6, was inhibited by the specific FcγⅡa receptor antibody.Conclusions In vitro, CRP increases the production of IL-6 in hPASMCs mediated by FcγⅡa receptor and NF-κB translocation. These data offer important insights into the role of CRP in the pathogenesis of PAH.  相似文献

Objective To study the efficacy of trimctazidine combined with atorvastatin for primary hypertension with paroxysmal auricular fibrillation,and its effects on LAD and CRP. Methods 160 patients of pri-mary hypertension with paroxysmal auricular fibrillation were randomly divided into 4 groups. Forty patients were treated with amiodarone (control group),600 mg/d for the first week,400 mg/d for the second week and 200 mg/d later;40 patients were treated with atorvastatin (20 mg/d,3 times per day) in addition to amiodarone (the atorvasat-in group);40 patients were treated with trimetazidine (20 mg/d,3 times per day) in addition to armiodarone (the trimetazidine group);40 patients were treated with combination of trimetazidine and atorvastatin in addition to amiod-atone (the combination group),and the dose was the same as the above groups. The treatment was started within 24 hours of recovering from paroxysmal auricular fibrillation and lasted for 1 year. Results After 1 year there was 1 pa-the control group,and 62.5% (25/40) for the atorvasatin group,64.1% (25/39) for the trimetazidine group,and 84.6% (33/39) for the combination group. Compared to the control group,the effective rate of the 3 treatment groups were all significantly higher (X2=4.56、5.13、17.55,P<0.05). The effective rate of the combination group was significantly higher than that of the atorvasatin group and the trimetazidine group (X2=4.95、4.30,P<0.05),and there was no significant difference of effective rate between the atorvasatin group and the trimetazidine group(X2= >0.05). After treatment LAD was (40.96+1.81) mm in the control group,(38.65±1.90) mm in the atorvasatin group,(39.15±1.85)mm in the trimetazidine group,and (37.22±1.74) mm in the combination group. LAD of the 3 treatment groups were all significantly different from the control group(F=3.42,P<0.05). LAD of the combina-tion group was significantly smaller than that of the atorvasatin group and the trimetazidine group (P<0.05),and there was no significant difference of the LAD between the atorvasatin group and the trimetazidine group(P>0.05). There was no significant difference between the 4 groups on CRP before treatment (F=0.96,P>0.05). After treat-ment CRP was (8.85±1.45) mg/L in the control group,(5.96±1.26) mg/L in the atorvasatin group,(6.81± 1.37) mg/L in the trimetazidine group,and (3.75±1.15) mg/L in the combination group. CRP of the 3 treatment groups were all significantly different from the control group (F=3.63,P<0.05). CRP of the combination group was significantly lower than that of the atorvasatin group and the trimetazidine group (P<0.05),and there was no signif-icant difference of CRP between the atorvasatin group and the trimetazidine group (P>0.05). Conclusion The treatment with trmetazidine combined with atorvastatin could prevent recurrence of paroxysmal auricular fibrillation though anti-inflammatory and inhibiting the remodeling of left atrial.  相似文献
