Objective To study the anastomotic relationships of perforators in each zone of the poste-rior leg and design perforating flaps for clinic. Methods Six fresh cadavers underwent a whole body, intra-arterial injection of a lead oxide and gelatine preparation. The posterior part of leg is divided into upper, mid-die and below equal parts, Observe topography of the perforating branches in every district by layer, and mea-sured their location, diameter, course, branches and anastomosis pattern. Radiographs of tissue specimens were digitally analyzed. Results There were 13 perforators that diameter≥ 0.5 mm, the average external diameter was 0.8 mm. The areas of each perforator supplied was average 38 cm2. Perforators from popliteal artery was identified an area 4 cm wide, around the intersection of two lines, a line drawn between the medial and lateral epicondyles of the femurs, and the midline of posterior leg. The areas of the every perforator sup-plied was 55 cm2. These vessels were large in diameter and create multiple true anastomoses with the perfora-tors from the posterior tibial artery or fibular artery. Perforating branches were small in the below part, a long perforator chain comprised of two to three perforators accompanies the Achilles tendon. Conclusion The perforator flaps deviced by perforators from the posterior leg may be transplanted to the lower limbs and the other part of the body. The perforators located in the middle zone of the leg are larger. Free posterior tibial or peroneal perforator-based flaps are reliable, relatively large, and have thin flaps. 游离胸背动脉穿支皮瓣桥式移植修复小腿软组织缺损   总被引:1,自引:1,他引:0  
目的总结游离胸背动脉穿支皮瓣或肌瓣桥式移植修复小腿软组织缺损的临床应用效果。方法自2006年9月至2009年1月,应用游离胸背动脉穿支皮瓣或肌瓣桥式移植修复小腿软组织缺损11例,缺损范围4cm×8cm至8cm×22cm。皮瓣切取连带肩胛下与旋肩胛血管,血管蒂呈T形,与健侧小腿胫后动脉行端端吻合,血管蒂用中厚网状游离植皮覆盖。结果除1例术后皮瓣远端发生小的表浅感染,经换药后愈合外,本组皮瓣全部成活。术后随访9个月至3.6年(平均2.9年),没有发现明显的供区功能障碍,供区与受区外形较好,健侧小腿经临床观察与Doppler检查,胫后动脉通畅。结论本方法适用于修复四肢软组织缺损后,患者仅存1条主要动脉者;行桥式游离胸背动脉穿支皮瓣或肌瓣移植不损伤健侧小腿胫后动脉,降低了对供区的损伤。  相似文献

目的 介绍改进桥式交叉游离皮瓣移植的手术方法并总结分析.方法 2003年9月至2007年5月,应用桥式交叉"T"形血管吻合游离背阔肌皮瓣移植方法修复小腿软组织缺损患者6例.男5例,女1例;年龄21~48岁(平均32岁).软组织缺损范围20 cm×9 cm~32 cm×11 cm.皮瓣血管蒂切取时携带肩胛下与旋肩胛血管,使蒂呈"T"形,将其与健侧小腿胫后动脉两断端行端端吻合,血管蒂行中厚网状游离植皮包裹,未用皮管,小腿供区切口直接缝合.结果 1例术后皮瓣远端发生小的表浅感染,换药2周后愈合,皮瓣全部成活,术后经过顺利,获得较满意的效果.随访6个月~4年(平均2.7年),未发现明显的供区功能障碍.供区与皮瓣外形较好,供区小腿经临床观察与多普勒检查证实胫后动脉通畅.结论 利用"T"形血管与胫后血管吻合,降低对侧小腿供区的损伤,只要血管吻合质量好,对皮瓣血供无影响.这种技术特别适用于小腿软组织缺损须行血管吻合的皮瓣修复、但肢体仅有一根主要血管的病例.  相似文献

目的探讨应用胫后动脉穿支皮瓣修复小腿及足踝部皮肤软组织缺损的新方法。方法应用非恒定蒂胫后动脉链式穿支皮瓣修复小腿及足踝部皮肤软组织缺损32例,术前应用多普勒血流仪探测胫后动脉皮支位置,利用各皮支形成的链式供血方式,选择距创面直线距离最近的皮支点做为旋转点切取皮瓣修复创面。结果经1.5个月~2年的随访,所有皮瓣均成活良好,外观满意,无手术并发症。结论非恒定蒂胫后动脉链式穿支皮瓣不受蒂部位置的限制,切取灵活方便,符合以最小的供区代价换取最佳的修复效果的基本原则,是一种修复小腿及足踝部皮肤软组织缺损的理想术式。  相似文献

目的探讨小腿内侧皮瓣联合内侧半比目鱼肌瓣桥式带蒂转移术治疗对侧小腿软组织缺损的疗效。方法回顾性分析2012年1月至2016年1月甘肃省兰州市兰州手足外科医院骨科收治的8例小腿软组织缺损患者资料。男5例,女3例;年龄19~50岁,平均35岁。所有患者均以胫后动脉为蒂小腿内侧皮瓣联合内侧半比目鱼肌瓣桥式带蒂转移术治疗。软组织缺损的范围为10 cm×9 cm^13 cm×8 cm。肌瓣和血管蒂表面行一期中厚网状游离植皮,供区直接缝合。末次随访时按Iowa等提出的胫骨骨折疗效评定方法评定疗效。结果术后所有患者的皮瓣和肌瓣全部成活,没有发生血管危象。有1例患者发生肌瓣远端小块植皮坏死,经2周换药处理自然愈合。术后所有患者获2.5~4.5年(平均3.8年)随访。受区外形较好。末次随访时按Iowa等提出的胫骨骨折疗效评定方法评定疗效:优3例,良4例,可1例。结论小腿内侧皮瓣联合内侧半比目鱼肌瓣桥式带蒂转移术适用于治疗小腿仅有1条主要血管的软组织缺损,可减轻供区损伤。  相似文献

A 48-year-old man sustained a traffic accident injury to his left leg. It was an open fracture of the left tibia and fibula accompanied by a large soft tissue defect (27 cm×7 cm). Doppler examination revealed the posterior tibial artery was occluded due to thrombosis. Three weeks after injury, the latissimus dors myocutaneous flap was elevated with a T-shaped vascular pedicle and was interposed between the two vascular ends of the posterior tibial vessel of the contralateral leg. Two end to end anastomoses were performed between the two vascular ends of the posterior tibial vessel of the contralateral leg and the latissimus dors myocutaneous flap’s T-shaped vascular pedicle. The latissimus dorsi myocutaneous flap was used for repair of a large soft tissue defect of the left leg. The vascular pedicle was cut off after 28 days and the flap survived completely. After 3-years’ follow-up postoperatively, a good contour was confirmed at the recipient area. The right tibia and fibula fractures were confirmed healing radiologically. The posterior tibial artery of contralateral leg was demonstrated patent by clinical and Doppler examinations.  相似文献

This article reports 2 cross-leg free composite tissue flaps for repairing the severe composite tissue defects in lower leg without suitable adjacent recipient vasculature for microvascular anastomosis. The osseous myocutaneous flap of ilium and tensor fascia lata pedicled with ascending branch of lateral femoral circumflex vessels and the osseous muscle flap of scapula and latissimus dorsi pedicled with subscapular vessels were performed, respectively, to reconstruct the bone and soft-tissue defects in the lower leg of 2 patients. Both donor vessels were the posterior tibial artery and great saphenous vein from the contralateral lower leg. The legs and the bone flaps were immobilized by an external fixator. The periods of pedicle division were 43 and 67 days, respectively, after transplantation. Both flaps survived after pedicle division and the patients regained the ability to walk. There were no such complications as joint stiffness or donor site morbidity except for a linear scar. The 2 cross-leg free composite tissue flaps were optional methods for salvaging limbs that were otherwise nonreconstructable. But the indication for cross-leg free-tissue flap should be limited strictly.  相似文献

目的观察逆行供血的吻合血管胸脐皮瓣修复胫骨外露创面的临床效果.方法对44侧小腿动脉铸型标本进行了观测的基础上,于1993年5月~2000年7月,对临床收治的25例小腿中上段大面积软组织缺损,胫骨外露,其中18例有胫前或胫后血管长段受损栓塞,创面最大为28 cm×11 cm,最小为11 cm×9 cm.采用小腿远端胫前或胫后血管远端与腹壁下血管吻合的胸脐皮瓣移植修复创面.结果胫前动脉、胫后动脉和腓动脉三组主干型血管,三者彼此间在足踝部交通吻合十分丰富,有很好的相互代偿能力.术后移植皮瓣全部成活,皮瓣色泽、质地良好,创面完全修复.结论只要小腿胫前或胫后主要血管之一正常,受损血管远端正常,逆行供血良好,即可采用吻合血管的胸脐皮瓣修复小腿中上段大面积软组织缺损.  相似文献

Between 2001 and 2003, 30 serratus anterior free flaps have been realized in our unit for reconstruction of lower limb. The flaps were indicated for repair of traumatic soft tissue defect in 19 cases, for chronic wound in six cases, for purpura fulminans in two cases, for pressure sore in one case and after tumor resection in 1 case. There were 29 patients: 25 males and four females. They ranged in age from 5 to 64 years. The size of the soft tissue defects ranged between 15 and 180 cm(2). Four different types of serratus anterior flaps were used: 20 muscle flaps ; four myocutaneous flaps ; five costo-osteomuscular flap ; one costo-osteomyocutaneous flap . In all cases we used osteo flap (8 th rib) in order to restore bone defect, which ranged from 9 to 15 cm. Necrosis occurred in two flaps because of venous thrombosis. The functional outcome was good for all patients and the aesthetic sequelae at the donor site were considered as minor. The serratus anterior flap can be used in many different ways: different kinds of flaps (osteo-cutaneous-muscle); very variable size of flap (15 to 180 cm(2) in our series) , different length of pedicle possible. Because of his versatility and his absence of major functional or aesthetic sequelae the serratus anterior free flap has become day by day our favourite option in limb reconstruction.  相似文献
