| 摘 要： | 目的 探讨影响pT4aN0M0期胃癌预后的相关因素。方法 回顾性分析57例行根治性手术治疗且术后病理分期为pT4aN0M0的胃癌患者临床病理资料，比较病理特征与临床预后的关系。结果 pT4aN0M0期胃癌患者发病年龄多≥60岁，男性多于女性，BMI多低于24 kg/m2，肿瘤部位多位于胃中上部；肿瘤大小≥5 cm、阴性淋巴结数目≥15个、术后辅助化疗、肿瘤分化程度与pT4aN0M0期胃癌预后相关（r=0.266，0.478，0.971，0.515，P<0.05，）；COX模型多因素分析显示，肿瘤大小、阴性淋巴结数目、术后辅助化疗、肿瘤分化程度是影响其预后的独立危险因素（P<0.05）。结论 肿瘤大小、阴性淋巴结数目、术后辅助化疗、肿瘤分化程度可能与pT4aN0M0期胃癌患者预后有关，其中术后辅助化疗是最重要因素。

| Abstract: | Objective To study the factors affecting prognosis of pT4aN0M0 period gastric cancerfor providing a reference for cancer prevention. Methods A retrospective analysiswas performed on the clinical data of 57 patientswith postoperative pathologic stage pT4aN0M0 and who underwent radical surgery from January 2009 to January 2011, the five-year survival rate was calculated and comparedusing univariate and multivariate. Results pT4aN0M0 gastric cancer patients aged more than 60 years, there were more men than women, BMI was mostly less than 24 kg/m2, and the tumor sites were located in the upper part; tumor size was larger than 5 cm, negative lymph node number was more than 15, and adjuvant chemotherapy, tumor differentiation and prognosis of pT4aN0M0 gastric cancer were relevant. Meanwhile, gender, age, BMI and prognosis of pT4aN0M0 gastric cancer showed no relevance,and COX multivariate analysis showed that tumor size, number of lymph node-negative, adjuvant chemotherapy, tumor differentiation were the independent risk factors affecting the prognosis. Conclusion Tumor size, lymph node-negative number, adjuvant chemotherapy, tumor differentiation may be associated with the prognosis of pT4aN0M0 gastric cancer. |
