急性呼吸道感染患儿中不同基因型人鼻病毒感染研究

摘    要:目的 了解急性呼吸道感染住院患儿中不同基因型人鼻病毒(Human Rhinovirus,HRV)流行情况,并进行遗传变异分析。 方法 采集2010年1月至2014年2月年成都某妇女儿童医院因急性呼吸道感染住院患儿的鼻咽抽提物1 780份,采用巢式PCR方法,先用HRV-5′NCR引物筛检标本,了解该病毒总检出率,阳性标本再用HRV-VP4/VP2引物进行分型检测,利用MEGA6.06软件分析其变异情况。 结果 在1 780份鼻咽抽吸物标本中,HRV阳性为365例(20.5%,365/1 780),其中244份VP4/VP2基因阳性,经序列分析发现132例为A型、30例为B型、82例为C型;不同基因型的检出率在月份和年龄上有一定波动,其中A型和C型呈交互流行特点;经同源性分析发现不同基因型内和型间的核苷酸同源性差异较大。 结论 HRV是成都地区急性呼吸道感染较为常见的病原之一,以HRV-A型和C型流行为主,各个基因型间具有较高的变异性。

Abstract:Objective To investigate the prevalence of different groups of human rhinovirus (HRV) infection in hospitalized children with acute respiratory tract infections (ARI),and to analyze the genetic characters of VP4/VP2 gene of HRV. Methods 1780 nasopharyngeal aspirates (NPA) samples were collected regularly from the children with ARI,from Women and Children′s Hospital in Chengdu area between Jan.2010 and Feb.2014.Samples were screened for the positive rate of HRV-5′NCR gene with nested-PCR,then the VP4/VP2 gene amplified from HRV positive specimens was sequenced for HRV genotype,meanwhile analyzed its genetic variation using MEGA6.06 software. Results A total of 20.51% (365/1780) of these samples were positive for HRV.244 sequences (positive for the VP4/VP2 gene) were obtained which included 132 HRV-As,30 HRV-Bs and 82 HRV-Cs.Different groups of HRV had various detectable rates in months and age,and different but interactive patterns of HRV-A and C circulation were identified.Phylogenetic analysis revealed that HRV strains had a higher difference in homologies of nucleotides. Conclusion HRV is a relatively common pathogen in children with ARI in Chengdu area.The results suggest that HRV-A and C are the main subtypes,and different genotypes have high genetic diversity.
