乳腺微小病变的X线及MRI对比研究

目的:对比分析乳腺微小病变MRI与X线征象，提高恶性病变早期检出率及诊断率。方法:回顾性研究94例乳腺患者共176处微小病灶的X线、MRI资料，对照影像诊断与病理结果，分析两种方法对乳腺微小病变的诊断价值。结果:病理良性病灶131处、恶性45处，两种检查手段的敏感性、特异性、准确度、阳性预测值、阴性预测值如下:X线71.11%、86.25%、82.38%、64.0%、89.68%；MRI:93.33%、95.41%、94.88%、87.5%、97.66%，X线与MRI诊断乳腺微小病变准确率差异存在统计学意义（χ2检验，P＜0.05）。结论:乳腺微小病变MRI诊断准确性高于乳腺X线摄影，DCE-MRI对早期无肿块型乳腺癌的诊断及敏感性、特异性较高，联合应用有助于提高早期检出率及诊断准确率。

The X-ray and MRI features of small breast lesions

Objective:To improve the preoperative diagnostic accuracy by studying the X-ray and MRI features of small breast lesions.Methods:The X-ray and MRI features of 94 patients with 176 focus confirmed by pathology were studied retrospectively and compared.Results:The pathological results of the whole group were as follows:131 benign lesions and 45malignant ones.The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of MRI were as follows:93.33%,95.41%,94.88%,87.5%,97.66%.The above values are higher than X-ray :71.11%,86.25%,82.38%,64.0%,89.68%.There were statistically significant difference of sensitivity,specificity and diagnostic accuracy between X-ray and MRI (χ2 test,P＜0.05).Conclusion:MRI has high sensitivity and specificity in the early diagnosis of small breast lesions,occult breast cancer.The diagnostic accuracy is higher than X-ray.Comprehensive analysis of MRI and X-ray is important for accurate preoperative diagnosis.
