摘    要:为了弄清脊髓损伤的发展规律,我们对动物模型从Allen的背侧撞击改用腹侧撞击,使其更接近于脊髓损伤的模式。在一定功率下可使动物模型产生可逆性单侧损伤,从而观察脊髓损伤后的病理变化。用0.0158J/cm的力,制成不全截瘫的动物模型,并用SEP波型及其潜伏值来进行定量分析,了解伤后的发展进程,同时通过病理观察;与SEP检查匹配,对脊髓损伤的程度和变化有一个比较清楚的概念。对脊髓损伤,完全性操作不会出现SEP波,一旦SEP波出现,说明脊髓功能已存在,单从波形角度考虑,不能说明可以恢复,应看其发展过程中潜伏值的变化,比较上、下肢的潜伏值系数,才具有显著的评估意义。可以看出潜伏值的变化与临床表现有密切关联。SEP只能代表脊髓的感觉功能,而不能代表运动功能。

Abstract:Alien has developed an experimental model of spinal cord injury through dorsal route since 1981. However his approach didn' t imitate the real mechanism of spinal cord injury of ventral impaction. Therefore, we modified his approach to ventral contusion instead of dorsal impaction. We adjusted, the force of 0. 0158 J/cm as an adequate force to contuse the spinal cord, so that a reversible injury could be modeled. During recovery, we could follow the sequence of improvement precisely. Therefore, the model of ventral approach could be used as a supplement of Alien technique in studying the pathogenesis of spinal cord injury. The SEP wave pattern, SEP latency examination and the coefficient of latency comparing the lower limb with the upper limb are used for evaluation. We consider that the latter two measures are more significant in tracing the injury in order to evaluate the functional recovery of spinal cord injury and root lesions. As SEP latency examination, the comparison of reading of the affected side with the normal is significant. Diagnosis could be made if the SEP latency caused by stimulating the area which was innervated by the injured nerve was above the maxinal normal reading. The coefficient of latency could reflect the stage of recovery of spinal cord injuries. However, SEP can only concern the sensory function of tile spinal cord, but not the motor function.
