腹腔镜手术和开腹手术在早期卵巢肿瘤分期探查术中的应用效果 及术后并发症比较

目的比较腹腔镜手术和开腹手术在早期卵巢肿瘤分期探查术中的手术效果及术后并发症。方法回顾性分析118例行腹腔镜手术或开腹手术的早期卵巢肿瘤患者的临床资料,包括一般资料、手术时间、失血量、淋巴结数目、术后排气时间、术后住院时间、术后疼痛和并发症。结果腹腔镜手术组手术时间长于开腹手术组(P<0.05),术中出血量少于开腹手术组(P<0.05),术后排气时间、住院时间短于开腹手术组(P<0.05),术后6、24、48 h疼痛评分均显著低于开腹手术组(P<0.05)。腹腔镜手术组术中输血率显著低于开腹手术组(P<0.05);2组手术中切除淋巴结的数目比较差异无统计学意义(P>0.05);腹腔镜手术组围手术期并发症发生率显著低于开腹手术组(P<0.01)。结论腹腔镜手术可成功完成早期卵巢肿瘤分期探查术,且术中出血量少,术后排气时间和住院时间短,术后疼痛轻,并发症少。

Comparison of effect and postoperative complications between laparoscopic operation and laparotomy for early ovarian tumor staging exploratory surgery

Objective To compare the effect and postoperative complications between laparoscopic operation and laparotomy for early ovarian tumor staging exploratory surgery.Methods The clinical data of one hundred and eighteen patients with early ovarian tumor who underwent laparoscopic operation and laparotomy were retrospectively analyzed,including general state,operation time,blood loss,lymph node count,postoperative anus exhaust time,postoperative hospital stay,postoperative pain and complication.Results Compared with laparotomy group,in the laparoscopic operation group,the operation time was longer(P<0.05),intraoperative blood loss was less(P<0.05),postoperative anus exhaust time and postoperative hospital stay was shorter(P<0.05),the postoperative pain scores were significantly lower after 6,24 and 48 hours(P<0.05),the intraoperative blood transfusion rate and complication incidence were significantly lower(P<0.05).There was no statistically significant difference in the number of surgical removal of lymph nodes(P>0.05).Conclusion Laparoscopic operation can be successfully completed in the early ovarian tumor stage exploratory surgery,and with less blood loss,shorter postoperative exhaust time and hospital stay,mild postoperative pain,and few complications.
