胰岛素泵联合持续动态血糖监测系统治疗初诊2型糖尿病疗效观察

摘    要:目的探讨应用胰岛素泵胰岛素皮下持续输注(CSII)联合持续动态血糖监测系统(CGMS)治疗初诊2型糖尿病(T2DM)的疗效。方法 60例初诊T2DM患者分为指尖自我血糖监测(SMBG)组和CGMS组,每组30例;2组患者均采用胰岛素泵治疗,治疗后对2组患者的疗效进行比较。结果 2组治疗前血糖水平比较差异无统计学意义(P>0.05);治疗后2组患者血糖水平均显著低于治疗前(P<0.05);治疗后CGMS组患者空腹、早餐后2 h、午餐后2 h、凌晨200、晚2200血糖水平均显著低于SMBG组(P<0.05);治疗后2组晚餐后2 h血糖比较差异无统计学意义(P>O.05)。2组患者血糖达标时间比较差异无统计学意义(P>0.05);CGMS组患者血糖达标所需胰岛素用量少于SMBG组(P<0.05)。CGMS组患者低血糖发生次数少于SMBG组(P<0.05)。2组治疗后患者胰岛β细胞功能指数均高于治疗前(P<0.05);治疗后CGMS组患者胰岛β细胞功能指数高于SMBG组(P<0.05)。结论胰岛素泵联合CGMS治疗T2DM能有效控制血糖,改善胰岛β细胞功能,减少低血糖发生。

Abstract:Objective To explore the effect of insulin pump combined with continuous glucose monitoring system(CGMS) on patients with primary diagnostic type 2 diabetes mellitus.Methods Sixty patients with primary diagnostic type 2 diabetes mellitus were divided into self-monitoring of blood glucose(SMBG) group and CGMS group,thirty cases in each group.The patients in two groups were treated with insulin pump,the curative effect was compared between two groups after treatment.Results There was no significant difference in the blood glucose level between two groups before treatment(P>0.05).The blood glucose level after treatment was significantly lower than that before treatment in two groups(P<0.05).The blood glucose levels of fasting,2 hours after breakfast,2 hours after lunch,2:00 am and 22:00 pm in CGMS group were significantly lower than those in SMBG group(P<0.05);but there was no significant difference in the blood glucose level of 2 hours after dinner between two groups(P>0.05).There was no significant difference in the blood glucose recovery time between two groups(P>0.05).The insulin dosage in CGMS group was less than that in SMBG group when the blood glucose recovery(P<0.05).The hypoglycemia occurrence in CGMS group was less than that in SMBG group(P<0.05).The HOMA-Iβ after treatment was significantly higher than that before treatment in two groups(P<0.05).After treatment,the HOMA-Iβ of CGMS group was significantly higher than that of SMBG group(P<0.05).Conclusion Insulin pump combined with CGMS for the treatment of T2DM can effectively control blood glucose,improve bata cell function of islets of pancreas,and reduce the incidence of hypoglycemia.
