中医证候量表在广泛性焦虑障碍患者应用中的信度和效度研究

目的探讨中医证候量表(TCMDS)应用于广泛性焦虑障碍患者的信度和效度。方法对227例广泛性焦虑障碍患者进行TCMDS和汉密尔顿焦虑量表(HAMA)评分。结果 TCMDS各条目及总量表的Cronbach'sα系数为0.72～0.81;评分者TCMDS的一致信度kappa系数为0.86。TCMDS总分与HAMA总分的spearman相关系数为0.87(P<0.000 1);TCMDS在判断临床疗效方面具有较高的灵敏度和特异性,曲线下面积在判断疗效方面均大于0.86(P<0.000 1)。TCMDS的前5个条目的累计贡献率为0.39～0.76,均高于HAMA。结论 TCMDS具有良好的信效度,可以应用于广泛性焦虑障碍患者的焦虑症状评估。

Reliability and validity of traditional Chinese medicine dialectical scale for generalized anxiety disorder

Objective To explore the reliability and validity of traditional Chinese medicine dialectical scale(TCMDS) applied in generalized anxiety disorder. Methods Two hundred and twenty-seven patients with generalized anxiety disorder were asked to complete the TCMDS and Hamilton anxiety scale(HAMA). Results Cronbach′s α coefficients and scorer coefficients for the subtest ranged from 0.72 to 0.81.Kappa coefficient of the inter-rater reliability for categorization was 0.86.Correlation coefficient between TCMDS and HAMA total score was 0.87(P<0.000 1).In evaluating clinical efficacy way,TCMDS had high sensitivity and specificity and area under curve was more 0.86(P<0.000 1).The cumulative contribution of the first five items in the TCMDS was 0.39 to 0.76,higher than HAMA. Conclusion TCMDS has good reliability and validity,can be applied to evaluate the dysphoric symptom in patients with generalized anxiety disorder.
