红细胞压积对急性胰腺炎患者预后的评估价值

目的了解红细胞压积对急性胰腺炎患者病情评估的意义。方法对329例急性胰腺炎患者的临床资料进行了回顾性分析,了解其红细胞压积变化与疾病的严重度及预后关系。结果HCT值判断重症胰腺炎的敏感性、特异性、分别为45.2%和64.3%,阳性预测值与阴性预测值分别为19%和86.9%,轻型胰腺炎的HCT值(中位数值为36.8%)与重症急性胰腺炎的HCT值(中位数值为43.1%)比较差异有统计学意义(P<0.01)。结论HCT值可作为重症急性胰腺炎有效而简便、经济的早期预测指标,HCT值不高者发生重症胰腺炎的可能性亦随之降低。

THE predictive value of Hemoconcentration in assess prognosis of acute pancreatitis

Objective To investigate the hematocrit(hct) levels in acute pancreatitis and analyze Hct predictive values in assessment acute pancreatitis.Methods Through retrospective analysis by 329 patients with a first acute pancreatitis episode treated at the affiliated hosp.of Ningxia med.coll from 2000 to 2004.we assess the dependability between hematocrit levels and prognosis.Results Hct levels in diagnosis SAP has value of sensitivity(45.2%)and specity(64.3%);the positive predictive value was19 % and negative predictive value was86.9 %;Hct median(36.8%)of mild acute pancreatitis was significantly lower as compared to severe pancreatitis(43.1%),(P<0.01).Conclusion Hct levels appears to be an early predictor of severe pancreatitis,the lower of HCT value the less possibility of SAP to patients.
