急性外伤性颅脑损伤患者凝血纤溶系统与其预后的相关性研究

目的:探讨急性外伤性颅脑损伤患者凝血纤溶系统与其预后的相关性。方法:选取110例急性外伤性颅脑损伤患者,根据格拉斯哥昏迷评分(GCS)分为两组(GCS>8分组和GCS≤8分组),同时选取100例健康体检者作为对照组。观察患者伤后24小时凝血酶原时间(PT)、凝血酶时间(TT)、活化部分凝血活酶时间(APTT)、纤维蛋白原(FBG)、D-二聚体(DD)及血小板计数(PLT)的变化。结果:与对照组比较,在伤后24h内,急性外伤性颅脑损伤患者除TT外,其余5项指标均有显著性差异(P<0.01)。GCS>8分组与GCS≤8分组比较,上述6项指标均有显著性差异(P<0.01)。结论:急性外伤性颅脑损伤后患者凝血纤溶系统有不同程度的变化,且与其预后密切相关。

Correlation between the Prognosis and Coagulation and Fibrinolysis System in Patients with Acute Traumatic Brain Injury

Objective:To evaluate the orrelation between the prognosis and coagulation and fibrinolysis system in patients with acute traumatic brain injury.Methods:110 patients with acute traumatic brain injury,according to Glasgow coma scale(GCS) were divided into two groups(GCS> 8 group,and GCS ≤ 8 group),and 100 healthy subjects were selected as control group.The change of plasma prothrombin time(PT),thrombin time(TT),activated partial thromboplastin time(APTT),fibrinogen(FBG),D-dimer(DD) and platelet count(PLT) was observed.Results:Compared with control group,within 24h after injury,the five indexes except TT of the patients with acute traumatic brain injury were significantly different(P<0.01) to the control group.The six indexes were significantly different(P<0.01) between the GCS> 8 group and the GCS ≤ 8 group.Conclusion:The coagulation and fibrinolysis systems in patients with acute traumatic brain injury have different degrees of change,and have closed relation with its prognosis.
