齐拉西酮合并艾司西酞普兰治疗以阴性症状为主的精神分裂症对照研究

目的 探讨齐拉西酮合并艾司西酞普兰治疗以阴性症状为主的精神分裂症的疗效和安全性。方法 将86例以阴性症状为主的精神分裂症患者随机分为研究组（齐拉西酮合并艾司西酞普兰治疗）和对照组（单用齐拉西酮治疗）,各43例。于治疗前及治疗后第2、4、8、12周末采用阳性和阴性综合征量表（PANSS）和阴性症状量表（SANS）评定两组的疗效,治疗中需处理的不良反应症状量表（TESS）评定两组的不良反应。结果 治疗后第2周末起研究组PANSS总分及阴性症状因子分较治疗前下降（P〈0.05）,研究组治疗后第2周末起PANSS总分及阴性症状因子分低于对照组（P〈0.05）。治疗后第4周末研究组SANS总分、情感平淡、注意障碍因子分低于对照组（P〈0.05）。两组不良反应发生率比较无显著性差异（P〉0.05）。结论 齐拉西酮合并艾司西酞普兰治疗以阴性症状为主的精神分裂症起效快、疗效好,且未增加不良反应。

A comparative study of ziprasidone combined with escitalopram in the treatment of schizophrenia dominated by negative symptoms

Objective To explore the efficacy and safety of ziprasidone combined with escitalopram in the treatment of schizophrenia dominated by negative symptoms.Methods 86 schizophrenic inpatients dominated by negative symptoms were randomized into study group （43 cases） treated with ziprasidone combined with escitalopram and control group （43cases） treated with ziprasidone monotherapy.All patients were assessed with Positive and Negative Syndrome Scale（PANSS） and Scale for the Assessment of Negative Symptoms （SANS） to evaluate the efficacy,and were assessed with Treatment Emergent Symptoms Scale （TESS） to evaluate the safety at baseline and at the end of the 2^nd,4^th,8^th and 12^th week of the treatment.Results Total score of PANSS and factor score of negative symptoms of PANSS in study group decreased significantly at the 2^nd week of the treatment when compared with the baseline （P 〈 0.05）,and were significantly lower than those in control group contemporarily （P 〈 0.05）.Total score of SANS and factor scores of apathy,attention dysfunction in study group were significantly lower than those in control group （P 〈 0.05）.There was no significant difference in incidence rate of side effects between the two groups （P 〉 0.05）.Conclusion Ziprasidone combined with escitalopram has rapid response,good efficacy and good compliance in the treatment of schizophrenia dominated by negative symptoms.
