华蟾素联合化疗EOF方案治疗晚期胃癌临床研究

目的:评估华蟾素联合化疗EOF方案治疗晚期胃癌的疗效和安全性。方法:选取经病理学证实的(手术47例,胃镜活检13例)晚期胃癌60例,随机分成2组,每组30例。两组均采用相同的EOF化疗方案,即奥沙利铂(L-OHP)135mg/m2d1,表柔比星(EPI-ADM)50mg/m2 d1加5-氟尿嘧啶(5-FU)375～425mg/m2 d1～5,维持120h,每3周重复,治疗组在此基础上,同时应用华蟾素20mL/d×10d;对照组(EOF)应用L-OHP 135mg/m2,EPI-ADM 50mg/m2 d1加5-FU 375-425mg/m2 d1～5,维持120h(静脉泵2mL/h),每3周重复,每化疗2个周期评价1次疗效。结果:治疗组30例中,完全缓解(CR)2例,部分缓解(PR)13例,稳定(SD)12例,进展(PD)3例,对照组3例中完全缓解1例,部分缓解12例,稳定13例,进展4例,治疗组治疗有效率(CR+PR)为50.0%,略高于对照组治疗有效率(43.3%)。中性粒细胞减少、贫血、恶心、呕吐、手足麻木发生率治疗组较对照组低,但血小板III～IV度下降发生率治疗组比对照组高,口腔炎、腹泻发生率2组无显著差异。结论:华蟾素注射液联合化疗EOF方案治疗晚期胃癌疗效较好,不良反应小。

Clinical Research on Huachansu with EOF Regimen in Patients with Advanced Gastric Cancer

Objective：To evaluate the safety and efficiency of Huachansu with EOF regimen in patients with advanced gastric cancer.Methods： A total of 60 patients with advanced gastric carcinoma diagnosed by pathology were enrolled,including 47 patients confirmed by operation and 13 patients confirmed by biospy.All patients were randomly divided into two groups.Patients in both groups were treated with oxaliplatin（135 mg/m2,2-h infusion,day 1）,epirubicin（50 mg/m2,day 1）,5-fluorouracil（375-425 mg/m2,day1-5,120-h infusion） every three weeks.And patients in the study group were given Huachansu 20 ml/d×10 d.The response rate was assessed after two chemotherapy cycles.Results： There were 2 complete response,13 partial response,12 stable disease and 3 progressive disease in the study group;one complete response,12 partial response,13 stable disease and 4 progressive disease in the control group.The beneficial rate of the study group was 50.0%,higher than that of the control group（43.3%）.Comparing the toxicity,the study group was superior to the control group on netropenia,anemia,nausea and vomit.The incidence of numb of limb in the study group was obviously lower than that in the control group.But the rate of thrombocytopenia was obviously higher in the study group than in the control group.There was no difference on oral ulcer and diarrhea between the two groups.Conclusions： The method of Huachansu with EOF regimen for advanced grastric carcinoma has good efficacy and safety.
