长期留置导尿管患者导管相关性尿路感染预防护理的最佳证据总结

目的评估并总结长期留置导尿管患者导管相关性尿路感染预防护理的最佳证据,为临床医务人员开展规范化的护理提供依据。方法检索美国国立指南库(NGC)、澳大利亚循证卫生保健中心(JBI)证据总结数据库、Cochrane数据库、英国国家卫生与临床优化研究所(NICE)、苏格兰院际指南网(SIGN)等所有长期留置导尿管导管相关性尿路感染预防护理的最佳证据,包括指南、系统评价、证据总结等。结果共纳入4篇文献,总结出预防长期留置导尿管导管相关性尿路感染预防护理的最佳证据54条,通过对证据进行归纳分析,最终形成长期留置导尿管患者尿管置管指征、减少尿道损伤、人员素质与要求、防止逆行感染、保持尿管系统密闭性和保持尿管系统通畅性6个方面的最佳证据。结论本次最佳证据总结为医务人员预防尿路感染的发生提供循证证据支持,起到预防长期留置导尿管患者导管相关性尿路感染发生的作用。

Summary of the best evidence for prevention and nursing of catheter-related urinary tract infections in patients with long-term indwelling catheters

Objective To evaluate and summarize the best evidence for the prevention of catheter-related urinary tract infection in patients with long-term indwelling catheters,and to provide evidence for clinical medical staff to carry out standardized nursing.Methods Search for the best evidence for preventive care for urinary catheter-related urinary tract infections,such as the national guide library(NGC),the Australian evidence-based health care center(JBI)evidence summary database,the Cochrane database,the national Institute for health and clinical excellence(NICE),and the Scottish inter-school guide network(SIGN),including guidelines,systematic reviews,and evidence summaries.Results A total of 4 articles were included,and 54 cases of prevention for prevention of long-term indwelling catheter-related urinary tract infections were summarized.The inductive analysis of the evidence finally resulted in the formation of long-term indwelling catheters for patients with urethral catheters.The best evidence for the levy,reduction of urethral injury,personnel quality and requirements,prevention of retrograde infection,maintenance of urethral system tightness and maintenance of urethral system patency.Conclusion This best evidence summary provides evidence-based evidence support for medical staff to prevent the occurrence of catheter-related urinary tract infections in patients with long-term indwelling catheters.
