短程正念认知团体人际疗法对成年早期抑郁障碍患者人际问题的干预研究

摘    要:目的探讨短程正念认知团体人际疗法对成年早期抑郁障碍患者及家属进行干预的效果。方法将60例符合入组标准的成年早期抑郁障碍住院患者按随机数字表法分为观察组和对照组各30例,两组在精神科临床路径下完成实验,对照组给予常规护理,观察组在常规护理的基础上进行正念认知团体人际疗法干预,两组分别在干预前、出院时和出院2个月时进行人际问题量表的测评。结果人际问题结果分析:入院时、出院时及出院2个月时两组患者的人际问题总分及其在主导/控制(PA)、竞争/自我中心(BC)、冷漠/距离感(DE)、社会逃避(FG)、顺从/不自信(HI)、过度接纳(JK)、自我牺牲(LM)、侵入性/过度表达(NO)得分在时间效应上差异有统计学意义(P<0.01),不考虑干预因素,观察组患者人际关系中的PA、 BC、 DE、 FG、 HI、 JK、 LM、 NO问题也会随时间的变化而改善;两组患者在分组效应上差异有统计学意义(P<0.01),即不同分组的患者人际问题及各维度得分有差异;组别效应和时间效应存在交互作用(P<0.01)。结论短程正念认知团体人际疗法对成年早期抑郁障碍患者人际问题有显著的改善作用。

Abstract:Objective To explore the effect of intervene early depressive patients and their family members through short-term minded cognitive group interpersonal therapy.Methods Sixty patients with depressive disorder in early adulthood were randomized into an intervention group(30 patients) and control group(30 patients). The two groups completed the experiment in the psychiatry clinical pathway. The intervention group conducted interpersonal therapy interventions of mindfulness cognitive groups on the basis of routine care. The two groups performed interpersonal problem scales before intervention, at discharge, and two months after discharge. Measurements of the scale. Results Analysis of interpersonal problem results: Total scores of interpersonal problems between the two groups of patients at the time of admission, at discharge, and 2 months after discharge were in PA(dominance/control), BC(competition/self-centered, DE(indifferent/distance Sense), FG(Social Evasion), HI(Submissive/unconfident), JK(Over acceptance), LM(Self-sacrificing), NO(Invasive/over-expressive) score analysis.There was significant difference in grouping effect between the two groups(P<0.01). The scores of interpersonal problems and dimensions of patients in different groups were different. There was interaction between group effect and time effect(P<0.01). Conclusion Short-term mindfulness cognitive group interpersonal therapy can significantly improve interpersonal problems of patients with early adult depression.
