摘    要:目的:探讨N-乙酰半胱氨酸(NAC)对大鼠急性坏死性胰腺炎(ANP)的作用及作用机制。方法:30只SD大鼠随机分为假手术(SO)组、ANP组、NAC组3组。采用胰胆管逆行注射4%牛磺胆酸钠(1 ml/1 kg)诱导ANP模型,NAC组于造模后30 min腹腔注射5%NAC(2 ml/kg)。造模后12 h取材,同时观察胰腺病理变化、大鼠血清淀粉酶(AMY)、丙二醛(MDA)、超氧化物歧化酶(SOD)、还原性谷胱甘肽(GSH)、谷胱甘肽过氧化物酶(GSH-PX)、过氧化氢酶(CAT)、胰腺组织髓过氧化物酶(MPO)及胰腺组织中核因子-κB(NF-κB)的表达。结果:ANP组胰腺病理损伤、AMY、MDA、MPO及NF-κB表达明显高于NAC组和SO组(P<0.01),NAC组这些指标显著低于ANP组(P<0.01),但高于SO组(P<0.01);GSH、GSH-PX、CAT在ANP组明显低于NAC组和SO组(P<0.05),经NAC处理后这些指标明显高于ANP组(P<0.05),但低于SO组(P<0.01);SOD在ANP组和NAC处理组无明显差异(P>0.05),但均低于SO组(P<0.01)。结论:NAC可通过抑制NF-κB活化和中性粒细胞聚集,减少氧自由基产生,减轻胰腺损伤,对ANP具有重要的治疗和保护作用。

Abstract:Objective: To investigate the effect of N-acetylcysteine(NAC) on acute necrotizing pancreatitis(ANP) and its mechanism.Methods: Thirty SD rats were randomized into sham operation(SO) group,ANP group and NACtreated group.The model of ANP was induced by retro-grade injection of 4% sodium taurocholate(1 ml per 1 kg body weight) into the bili-pancreatic duct.NAC-treated group was given 5% NAC(2 ml/kg) intraperitoneally 30 min after inducing SAP.All the animals were sacrificed at 12 h after induction of pancreatitis.The histopathological changes of pancreatic tissue were evaluated.The levels of amylase(AMY),malondialdehyde(MDA),superoxide dismutase(SOD),reduced glutathione hormone(GSH),glutathione peroxidase(GSH-PX),catalase(CAT) in serum,myeloperoxidase(MPO) in the pancreas and the expression of NF-κB in the pancreas were determined.Results: The histopathological changes of pancreatic tissue,the levels of AMY and MDA in serum,the activity of MPO and the expression of NF-κB in the pancreas were significantly increased in ANP group compared with those in SO group and NAC-treated group(all P<0.01),however,the indexes were significantly decreased after the using NAC(P< 0.01);the levels of GSH,GSH-PX and CAT in serum were significantly decreased in ANP group compared with those in SO group and NAC-treated group(P<0.05),however,the indexes were significantly increased after applying NAC(P<0.05),and no significant difference was found between ANP group and NAC-treated group(P>0.05),but all indexes in both groups were lower than in SO group(all P<0.01).Conclusion: NAC can restrain activated NF-κB,decrease pancreas leukocyte infiltration,and reduce the produce of the OFR and pancreas injury.NAC may have a therapeutic effect on ANP by eliminating oxygen free radicals.
