美托洛尔预防高血压病人气管拔管心血管不良反应的观察

目的:探讨美托洛尔注射液在预防高血压病人气管拔管心血管不良反应中的作用。方法:32例全麻高血压病人随机分为两组,美托洛尔组手术结束前约15分钟静注美托洛尔注射液0.05mg/kg,对照组静注生理盐水2～5ml,比较两组拔管前后血流动力学变化指标。结果:对照组在拔管前1min至拔管后10min与基础值相比心率显著增快,血压显著增高,美托洛尔组无明显变化,且两组间具有统计学差异。对照组在拔管前1min和拔管后1min与基础值相比心输出量显著增高,美托洛尔组无明显变化,且两组间具有统计学差异。结论:气管拔管可引起高血压病人血压增高、心率增快等心血管不良反应,拔管前静注美托洛尔0.05mg/kg具有明显的预防作用。

Evaluation of Metoprolol on cardiovascular response to tracheal extubation in patients with hypertension

Objective:To evaluate the role of Metoprolol injection in preventing the cardiovascular adverse effects of tracheal extubation in patients with hypertension. Methods:Thirty-two patients with hypertension who underwent general anesthesia were randomly assigned to intravenously inject with 0.05 mg/kg of Metoprolol (Metoprolol group) or 2 to 5 ml of normal saline (control group) 15 minutes before the end of the operation. The hemodynamic parameters were compared. Results:In control group,compared with the baseline,the heart rat significantly increased and the blood pressure was significantly higher 1 minute before and 10 minutes after the extubation;the cardiac output significantly increased 1 minute before and 1 minutes after the extubation. No such effects mentioned above were found in Metoprolol group. There were significant differences in heart rate,blood pressure,and cardiac output between Metoprolol group and control group. Conclusion:The adverse effects of tracheal extubation include increasing the heart and blood pressure in the patients with hypertension,which may be prevented by the intravenous injection of 0.05 mg/kg of Metoprolol.
