电视胸腔镜食管癌切除术20例报告

目的 总结电视胸腔镜食管癌切除术的临床应用经验。方法 1998年10月～2003年10月经电视胸腔镜行食管癌切除手术20例，2例作3～4个1．5cm的小切口，16例附加5～8cm长的小切口，1例附加10cm长的小切口完成食管游离及淋巴结清扫手术。1例中转开胸。结果 全组术中出血均在400ml以内。其中13例在200ml以内，术后发生颈部吻合口瘘1例，经保守疗法治愈。术后病理分期为：T2N0M05例，T2N1M03例，T1N2M02例，T3N0M07例，T3B1M02例，T3B2M01例，术后1、3、5年生存率分别为90％(18／20)，61％(11，18)和50％(2，4)。结论 电视胸腔镜可用于T1、T2和周围无粘连的T3期食管癌切除手术。

Video-assisted thoracoscopic esophagectomy for esophageal carcinoma

Objective To evaluate the clinical value of video-assisted thoracoscopic(VATS) esophagectomy for esophageal carcinoma. Methods 20 patients with esophageal carcinoma had been treated with VATS from October 1998 to October 2003. 19 patients were performed successfully with VATS,1 case was converted to thoracotomy becaus of bronchial artery hemorrhage.Results Anastomotic leakage occurred in one patient and was cured by non-operation method.All the patients were recovery after operation. The survival rate of 1,3 and 5 year was 90%,60% and 50% respectively. Conclusion VATS can treat the esophageal carcinoma which stage is T1,T2and some of T3 without adhesion .
