足月新生儿窒息脑利钠肽变化与NBNA评分的关系

摘    要:目的研究足月新生儿窒息患儿血浆脑利钠肽分泌水平的变化及其与NBNA评分的关系。方法选取2017年10月—2018年10月我院收治足月窒息新生儿172例,根据有无窒息将患儿分为对照组87例,轻度窒息组61例及重度窒息组24例,对比3组血清中脑利钠肽水平及新生儿行为神经评分。结果窒息组血清中BNP水平高于对照组,重度窒息组高于轻度窒息组(P<0.05)。不同时间段3组新生儿NBNA评分比较,对照组评分均高于轻度窒息组及重度窒息组,重度窒息组评分最低,差异有统计学意义(P<0.05)。血浆中BNP表达水平与第7、14、28天NBNA评分均呈负相关,差异有统计学意义(P<0.05)。结论BNP参与了窒息所致脑损伤病理生理过程,在疾病的早期诊断及病情评估方面具有重要的临床意义。

The Relationship Between the Changes of Brain Natriuretic Peptide and NBNA Score in Term Asphyxia

Abstract:Objective To study the changes of plasma brain natriuretic peptide(BNP)secretion and its relationship with NBNA score in term neonatal asphyxia.Methods From October 2017 to October 2018,172 newborns with full-term asphyxia admitted to our hospital were selected,and the children were divided into the control group(87 cases),the mild asphyxia group(61 cases)and the severe asphyxia group(24 cases)according to whether there was asphyxia or not.Results The level of BNP in asphyxia group was higher than that in control group,and that in severe asphyxia group was higher than that in mild asphyxia group(P<0.05).The NBNA scores of the three groups in different time periods were higher in the control group than in the mild asphyxia group and the severe asphyxia group.The scores of the severe asphyxia group were the lowest,the difference was statistically significant(P<0.05).The level of BNP expression in plasma was negatively correlated with NBNA score on the 7th,14th and 28th day(P<0.05).Conclusion BNP is involved in the pathophysiological process of brain injury caused by asphyxia,and has important clinical significance in the early diagnosis and disease assessment.
