闭合性十二指肠损伤外科治疗的探讨

目的:探讨闭合性十二指肠损伤诊断和手术方式的选择。方法:回顾性分析我院1998～2006年收治的32例十二指肠损伤诊治经过。结果:32例病人术前确诊8例,术中确诊24例,治愈28例,治愈率87.5%。手术后并发症出现14例,占43.8%,其中十二指肠瘘8例,占25%。结论:合理的手术方式是治疗十二指肠损伤的重要措施,早期手术和有效的十二指肠减压是影响预后的主要因素。

Study on surgical treatment for closed duodenal injury

Objective: To explore the diagnosis and surgical approach in patients suffering from duodenal injuries.Methods: The clinical data of 32cases of duodenal injury in our hospital from1998 to 2006 were summarized retrospectively.Results: Eight patients were diagnosed preoperatively,and 24 cases were diagnosed after operation.28 cases(87.5%) were cured.Complications occurred in14 cases(43.8%).Duodenal fistula was the main complication.Conclusion: Simple and reasonable surgical procedure plays a important role in management of duodenal trauma.Early operation and effective decompression of the duodenum are the major factors for prognosis.
