十六角蒙脱石和奥美拉唑合用治疗幽门螺旋杆菌感染的消化性溃疡

目的：探索十六角蒙脱石和奥美拉唑合用的抗幽门螺旋杆菌（ＨＰ）与抗消化性溃疡的疗效。方法：将８５例经胃镜及ＨＰ检查证实的ＨＰ阳性消化性溃疡病人随机分为３组。Ａ组３２例口服十六角蒙脱石３ｇ，ｔｉｄ，加奥美拉唑２０ｍｇ，ｑｄ。Ｂ组２８例服十六角蒙脱石３ｇ，ｔｉｄ。Ｃ组２５例服奥美拉唑２０ｍｇ，ｑｄ。疗程均为４ｗｋ。治疗前、疗程结束时及结束４ｗｋ后分别做胃镜和ＨＰ检查。结果：Ａ，Ｂ，Ｃ３组溃疡愈合率分别为９４％，７８％和８８％，Ｐ＞０．０５。ＨＰ根除率分别为６２％，３２％和４％，Ｐ＜０．０５或０．０１。结论：联合用药的ＨＰ根除率高于２药单独使用。但对消化性溃疡疗效，２药联合与单独用药无显著差别。

Dioctahedral smectite combined with omeprazole in treating peptic ulcer with Helicobacter pylori

AIM: To observe the eradicative effect of dioctahedral smectite (DS) combined with omeprazole on Helicobacter pylori (HP) and its efficacy in treatment of peptic ulcer. METHODS: Eighty-five patients with HP peptic ulcer diagnosed by gastroscopy and bacterial examination were randomly divided into 3 groups. Group A of 32 patients received DS, 3 g, po[WTBZ], tid and omeprazole 20 mg, po, qd for 4 wk; group B of 28 patients were treated with DS 3 g, po, tid for 4 wk; group C of 25 patients received omeprazole 20 mg, po, qd for 4 wk. Gastroscopic and bacterial examinations were made before right after and 4 wk after the end of therapy course to check the ulcer healing rates and the HP eradication rates. RESULTS: The ulcer healing rates in group A, B and C were 94%, 78%, and 88%, respectively (P>0.05). HP eradication rates were 62%, 32% and 4%, respectively P<0.05 or <0.01. In the 3 groups, the side effects were mild. CONCLUSION: The HP eradication rate of the combined therapy of DS and omeprazole was higher than that in a single drug, but there was no direct relation between HP infection and the healing of peptic ulcer.
