高原地区2型糖尿病患者微血管病变与血液学相关分析

摘    要:目的探讨高原地区2型糖尿病患者血液学指标水平变化与微血管病变程度关系。方法在海拔2260m以上青海西宁地区选择137例2型糖尿病患者和82例健康者进行血小板计数(PLT)、血小板平均体积(MPV)、血小板分布宽度(PDW)、尿微量白蛋白(U-mAlb)、血清Mg2+及糖化血红蛋白(HbAlc)测定,将两组结果进行比较分析。结果高原地区2型糖尿病组的HbAlc、PLT、MPV、PDW、U-mAlb、血清Mg2+的浓度显著高于正常对照组(P〈0.01)。随着HbAlc水平增高,PLT、MPV、PDW、U-mAlb水平随之增高,血清Mg2+有增高的趋势,呈显著正相关(P〈0.01)。结论2型糖尿病微血管病变程度与HbAlc的增高有关,血小板参数增高可作为血管及微血管病变的敏感指标,U-mAlb是糖尿病早期肾小球损伤的灵敏指标,微血管病变病情越重血清Mg2+水平越高。

Abstract:Objective To investigate the correlation of changes of hemotologial parameters with microvascular changes in type 2 diabetic patients in plateau areas.Method The levels of platelet parameters,platelet count(PLT),mean platelet volume(MPV),platelet distribution width(PDW),urinary albumin(U-mAlb),serum Mg2+ and glycated hemoglobin(HbAlc)in 137 type 2 diabetes patients and 82 healthy psersons in Xining,Qinghai province with a altitude of 2260m were determined and analyzed.Results The levels of HbAlc,PLT,MPV and PDW,U-mAlb serum Mg2+ concentration in type 2 diabetes group were significantly higher than that of the control group(P〈0.01).Along with the incease of HbAlc level,the levels of PLT/MPV/PDW/U-mAlb were increased and the serum of Mg2+was increased correspondingly.All these above mentioned indicators were significantly positively correlated(P〈0.01).Conclusion The change of microangiopathyis related with the change of HbAlclevel.The platelet parameterscan be used as a sensitive indicator for diagnosis of vascular and microvascular disease,and U-mAlb is also a sensitive indicator for identifying early diabetic glomerular lesions.The microvascular disease in type 2 diabetic patients will become more severe along with the increasing level of serum Mg2+.Combined detection is of great significance in early prevention,diagnosis and treatment of microangiopthy in type 2 diabetes.
