PTX-3对慢性心力衰竭患者心功能的评价及临床意义

目的探讨慢性心力衰竭患者血清正五聚蛋白-3（PTX-3）浓度对心功能的评价作用。方法选择住院的慢性心力衰竭患者130例,用酶联免疫吸附测定法检测受试者血清PTX-3浓度,超声心动图测定患者左心室射血分数及二尖瓣舒张早期流速峰/二尖瓣舒张晚期流速峰值。所得结果进行统计检验,比较不同类型心功能不全患者的PTX-3浓度,分析PTX-3浓度与不同类型心功能不全的相关性及其评估价值。结果左心室收缩功能不全患者血清PTX-3浓度显著高于左心室收缩功能正常患者（P〈0.01）,左心室舒张功能不全患者血清PTX-3浓度显著高于左心室舒张功能正常患者（P〈0.01）;Logistic回归分析显示,血清PTX-3浓度与左心室收缩功能不全、左心室舒张功能不全存在显著正相关（OR=2.12,P〈0.01;OR=2.10,P〈0.01）;不同NYHA心功能分级组之间比较,血清PTX-3浓度在各组间差异均具有统计学意义（P〈0.01）,且随心力衰竭严重程度的加重而明显增加。结论 PTX-3可以客观准确评价慢性心力衰竭患者的心功能,可作为一项临床诊断心功能不全的实验室指标。

The significance and assessment of Pentraxin-3 on cardiac function in patients with chronic heart failure

Objective To explore the role of serum pentraxin-3（PTX-3）levels on cardiac function in patients with chronic heart failure（CHF）.Methods 130 patients with CHF were enrolled in the study.The serum PTX-3 was determined by enzyme-linked immunosorbent assay.Left ventricular ejection fraction,mitral annular early diastolic and late diastolic flow velocities were measured by modified Simpson＇s equation with echocardiography.Results The serum PTX-3 concentrations of left ventricular systolic dysfunction patients were significantly higher than those of normal left ventricular systolic function（P0.01）and the serum PTX-3 concentrations in patients of left ventricular diastolic dysfunction were significantly higher than those of normal left ventricular diastolic function（P0.01）.Logistic regression analysis showed that the serum level of PTX-3 had significantly positive correlation with left ventricular systolic dysfunction or diastolic dysfunction（OR=2.12,P0.01 or OR=2.10,P0.01）.The differences of serum PTX-3 concentrations had statistical significance（P0.01）among all groups according to NYHA cardiac function increased with the increased severity of heart failure.Conclusions PTX-3 is a useful index for evaluation of cardiac function.
