TBLB在肾移植术后巨细胞病毒、肺孢子虫肺炎早期诊断中的意义

目的明确经纤维支气管镜肺活检（TBLB）在肾移植术后巨细胞病毒、肺孢子虫肺炎早期诊断中的意义。方法未行机械通气的患者可在密切监护下行TBLB，已行机械通气的患者可通过三通接口边通气边进行TBLB。结果6例经TBLB取活检组织后，再经免疫组织化学检查及六胺银染色后证实，4例为巨细胞病毒肺炎、2例为肺孢子虫肺炎。结论，TBLB在肾移植术后巨细胞病毒、肺孢子虫肺炎早期诊断中具有重要意义。

Significance of TBLB on the early diagnosis of patients with cytomegalovirus pneumonia and pneumocystosis after renal transplantation

Objective To identify the significance of transbronchial lung biopsy (TBLB) on the early diagnosis of patients with cytomegalovirus pneumonia and pneumocystosis after renal transplantation. Methods Adopted TBLB on the patients without mechanical ventilation in close monitoring.Used TBLB with mechanical ventilation via a special catheter (the lateral tap with one way plastic) mount without ceasing mechanical ventilation. Results Six patients took biopsy tissue by TBLB. It was confirmed through immunohistochemistry and silver staining. Four patients were diagnosed cytomegalovirus pneumonia and 2 pneumocystosis. Conclusion TBLB plays an important role in the early diagnosis of patients with cytomegalovirus pneumonia and pneumocystosis after renal transplantation.
