微创全髋关节置换治疗老年股骨颈骨折的手术分析

目的探讨微创全髋关节置换治疗老年股骨颈骨折的手术效果。方法将重庆市九龙坡区笫五人民医院2010年4月至2012年10月收治的100例老年股骨颈骨折的患者依据手术方式不同按随机数字表法分为两组,采用常规髋后外侧入路行人工全髋关节置换为人工关节置换组(50例);采用改良髋关节前外侧入路行微创全髋关节置换术为微创关节置换组(50例)。比较两组患者的临床疗效及不良反应情况。结果微创关节置换组治疗优良率显著高于人工关节置换组(92.0%vs 84.0%),微创关节置换组临床疗效优于人工关节置换组,差异有统计学意义(Z=3.120,P=0.017);微创关节置换组切口长度手术时间、术中出血量、24 h平均引流量均少于人工关节置换组(P<0.05),两组并发症发生率比较差异有统计学意义(P<0.05)。结论老年股骨颈骨折采用微创全髋置换术治疗可显著提高临床效果,改善患者生存质量。

Analysis on Minimally Invasive Total Hip Arthroplasty for the Treatment of Femoral Neck Fracturesl in the Elderly Patients

Objective To investigate the effect of the minimally invasive total hip replacement surgical treatment of femoral neck fractures in the elderly patients. Methods A total of 100 cases of elderly patients with femoral neck fracture admitted to the Fifth People＇s Hospital of Jiulongpo District from Apr. 2010 to Oct. 2012 ,were divided into the observation group and the control group, 50 patients in the control group were given conventional lateral approach total hip arthroplasty,50 patients in the observation group were given improved anterior approach minimallyoinvasive total hip arthroplasty. The clinical efficacy and adverse reactions of the two groups were compared. Results Excellent and good rate of observation group was significantly higher than the control group（92.0% vs 84.0% ） ,the difference was statistically significant（ Z = 3. 120, P =0. 017 ）;operative time, blood loss,24 h average drainage of the observation group were less than the control group （ P 〈 0.05 ） ; the incidence of complications of the two groups was statistically significantly different （ P 〈 O. 05）. Conclusion Femoral neck fractures in the elderly treated with minimally invasive total hin arthrnnlaty can significantlv imnrove the clinical outcomes and aualitv of life of the patients.
