重症监护室内患者细菌感染及耐药性分析

摘    要:目的了解枣庄市市中区人民医院2008年8月-2009年8月监护室内患者病原菌感染状况,为临床防治提供依据。方法使用LIS系统对2008年8月-2009年8月送检标本所分离的细菌及耐药特征进行回顾性调查分析输入内容。结果共统计标本563例,共分离致病菌株362株,其中革兰阴性菌有284株,革兰阳性菌45株,革兰阴性菌主要为铜绿假单胞菌,其余依次为大肠杆菌、肺炎克雷伯杆菌及鲍曼不动杆菌,革兰阳性菌以金黄色葡萄球菌为主,其余为肠球菌和凝固酶阴性的葡萄球菌。主要耐药菌为产β-内酰胺酶的大肠埃希菌、肺炎克雷伯菌,耐泰能的铜绿假单胞、鲍曼不动杆菌和耐甲氧西林的金黄色葡萄球菌。结论 ICU病房内病原菌感染以革兰阴性菌为主,大部分呈高度耐药性,因此做ICU病房内病原菌流行状况及耐药性调查对指导临床用药很有意义。

Abstract:Objective To understand pathogen infection in the patient care units of Central District People's Hospital of of Zaozhuang City from August 2008 to August 2009,to provide a basis for clinical prevention and treatment.Methods LIS systems was used to analyze characteristics of the bacteria isolated from the specimens and drug resistance for the retrospective.Results In total of 563 cases of statistical samples,362 strains of pathogens was isolated,including 284 gram-negative bacteria and 45 gram-positive bacteria,Gram-negative bacteria is mainly Pseudomonas aeruginosa,the rest of them are Acinetobacter baumannii,Klebsiella pneumoniae and Escherichia coli,gram-positive Staphylococcus aureus bacteria,enterococci and coagulase-negative staphylococci.The main drug-resistance bacteria was of Escherichia coli producing β-lactamases,Klebsiella pneumoniae and imipenem resistance in Pseudomonas aeruginosa,Acinetobacter baumannii,and methicillin-resistant Staphylococcus aureus.Conclusion Pathogen infection mainly related to Gram-negative bacteria and most of them with high drug-resistance in ICU wards.It is significant to know pathogens and its drug-resistance in ICU wards and guide clinical medication.
