十二指肠球部溃疡穿孔单纯修补术后疗效分析

进一步探讨十二指肠溃疡穿孔行单纯缝合术的后期疗效。方法 :结合文献分析了 88例十二指肠球部溃疡穿孔行单纯缝合术后的复发率及上腹部不适症状发生率、并发症率、再手术率。结果 :通过平均 10 3.3月的随访 ,远期复发率为 35 .2 % ,其中上腹部不适症状率 13.6 % ,并发症率 11.4% ,再手术率 10 .2 %。急性与慢性溃疡的复发率及再手术率有一定的差异 (P <0 .0 5 )。年龄及溃疡大小对术后复发率和再手术率均影响不大 (P >0 .0 5 )。结论 :十二指肠球部溃疡穿孔行单纯缝合修补术后辅以正规药物治疗疗效满意 ,预后良好 ,单纯修补术可作为十二指肠球部溃疡穿孔的主要治疗手段。

LATE RESULTS OF PERFORATED DUODENAL ULCER TREATED BY SIMPLE SUTURE

to explore the late results of perforated duodenal ulcer treated by simple suture. Methods:We analyzed ulcer recurrence rate, including ulcer symptom recurrence rate, complication rate and further operation rate 88 patients were surgically treated with simple closure, combined with the literature. Result: After a mean follow-up of 103.3 months, the overall recurrence rate was 35.2%, including ulcer symptom recurrence rate 13.6%, complication rate 11.4%, further operation rate 10.2%. The rate of recurrence and re-operation of acute ulcer is significant differ to that of chronic ulcer ( P <0.05), but these rate are not related to the age of patients or the size of ulcer ( P >0.05). Conclusions: Late curative effect of perforated duodenal ulcers is satisfied by simple closure with adjuvant regular medication. The prognosis is good. So the simple closure maybe a main treatment to perforated duodenal ulcers.
