小骨窗开颅"亚微创法"治疗高血压脑出血的临床研究

目的：探讨一种有效、微创的治疗高血压脑出血的方法。方法：按照CT定位避开脑功能区，于最近血肿处钻颅，再穿刺、引流并冲洗血肿。结果：全组18例术后即全部或大部分血肿清除7例，至拔管时(术后3—5d)血肿近全清除或大部分清除14例。随访3个月至3年，GOS优6例，良8例，中2例，差及死亡各1例。恢复优良率达78％(14／18)。结论：应用小骨窗开颅、综合亚微创法治疗高血压脑出血，具有简便、直接、微创和有效等优点；是比较理想的治疗高血压脑出血的措施。

The treatment of hypertensive cerebral hemorrhage by the drilling of the cranium using the small bone window

Objective: To explore an effective and mini-wound treatment of hypertensive cerebral hemorrhage. Methods: Avoiding the brain function area by CT's orientation, puncture, drainage and washing haematoma was carried out near haematoma spot. Results: Whole or most haematoma in 7 patients was cleared in 18 patients after surgery. At extubation (3 to 5 days later), whole or most haematoma was cleared in 14 patients. 3-month to 3-year follow-up, there was excellent 6 cases, good 8,better 2, bad 1 and death 1 according to GOS rating. Satisfied recovery rate was 78% ( 14/18 ). Conclusion: The treatment of hypertensive cerebral hemorrhage by the drilling of the cranium using the small bone window is simple, convenient, direct, mini-wounded, effective. This is an ideal cure for hypertensive cerebral hemorrhage.
