回肠代输尿管术

我院从1980～1998年共行回肠代输尿管术15例，随访11月～18年，全部健康生存，疗效满意。强调手术指征从严．当肾门区狭窄时，回肠袢近端与肾吻合原则是最低位、重度扩张的肾盏，以利引流通畅。回肠袢与膀胱吻合均采用直接吻合法，要求顺蠕动吻合，吻合口够大，并不强调在回肠袢末端作能抗逆流的乳头．回肠代输尿管术为处理复杂的上尿路梗阻性疾病、重建尿路的连续性提供了一个可行的良好方法。

Ileal Ureter

Ureter replacement with an isolated ileal segment was carried out between the year 1980 and 1998 in 15cases.Observed postoperatively for 11 months to 18years, all patients were alive and healthy. Satisfactory results havebeen achieved. Strictness with operative indications has been emphasized. When the renal hilus area was narrow,the ileal segment was anastomosed proximally to the seriously proliferated calyx at the lowes part of the kidneywhere the parenchyma was the thinnest, in order to facilitate adequate drainage. The distal end was anastomosedto the urinary baldder.Complicance with the direction of peristalsis was necessary for the anastomoticstoma,which was big enough to need no anti-renux nipple.Ileal ureter replacement provies a feasible way todeal with complicated upper urinary obstructive diseases and to restore urinary continuity.
