透明细胞癌样形态的黄色肉芽肿性肾盂肾炎1例报道及文献回顾

目的：探讨黄色肉芽肿性肾盂肾炎病理形态特征及鉴别要点以提高正确诊断率.方法：报道1例黄色肉芽肿性肾盂肾炎并结合文献对其组织学和免疫组织化学特点进行研究.结果：该例患者静脉肾盂造影检查提示：右侧肾盂结石、右肾功能消失；左侧肾盂轻度积水.病变区肾皮质的正常结构消失,代之大量的透明细胞成瘤样分布,伴有较多的淋巴细胞及浆细胞散在浸润.免疫组织化学染色显示透明细胞CD68阳性,RCC和KER阴性.结论：黄色肉芽肿性肾盂肾炎是一种比较罕见的炎性病变,根据其临床特点及组织学和免疫组织化学特征可以进行明确诊断.

Xanthogranulomatous pyelonephritis（XGP） ：1 case report and review of literature

Objective：To study the pathological characteristics of xanthogranulomatous pyelonephritis,in order to improve the correct diagnosis rate.Methods：One case of xanthogranulomatous pyelonephritis was reported and the pathological features were studied by microscopy and immunohistochemistry.Results：In the case,IVP （intravenous pyelography） showed pelvis calculus and disfunction of right kidney with hydronephrosis of left kidney.The normal structure of kidney cortex was destroyed and was replaced by foamy cells.Lymphocytes and plasma cells were disseminated among the foamy cells.Immunohistochemistry showed the fomay cells were positive for CD68,and negative for RCC and KER.Conclusion：Xanthogranulomatous pyelonephritis is a rare kidney disease and can be diagnosed by its characters of histology and immunohistochemistry.
