根管治疗期间急症的临床分析

目的 探讨根管治疗期间疼痛发生的原因以及预防和处理办法。方法 对714例1680颗患牙采用逐步后退法预备根管，并对根管治疗期间发生牙疼痛的原因进行综合分析。结果 根管治疗期间疼痛的发生率为13.6%，高发牙位为下后磨牙和下前牙，超充发生疼痛大于未超充的4倍，充填时症状未全消失者比症状消失者发生疼痛的概率显著增高（P〈0.05）。一次性根充术后发生疼痛者是两次或多次根充者的2.5倍。结论 根管治疗期间疼痛受多因素影响，也与术前患牙牙髓状况及充填时机、超充等有关，而根管预备彻底和规范操作是预防与处理的关键。

Clinical analysis of pain complicated during canal root therapy

Objective To explore the causes of pain complicated during canal root therapy as well as the prevention and treatment.Methods The canals of 1 680 teeth of 714 patients were prepared by using the step back technique.The causes of pain during the period of root canal therapy were comprehensively analyzed.Results The pain ratio complicated was 13.6% during canal root therapy.The pain ratio of mandibular posterior molar and mandibular anterior teeth was higher than that of the others.The pain ratio of the teeth suffered canal overfilling was 4 times more than that of teeth without overfilling canal.The pain ratio complicated of the canal being filled with some clinical symptoms was significant higher than that without clinical symptoms,P<0.05.The pain ratio complicated in the teeth with one-appointment root canal therapy was 2.5 times more than that with two-appointment root canal therapy.Conclusions The overfilling of the canal,unsuitable filling occasion and improperness of one-appointment root canal therapy are important causes of pain complicated during the canal root therapy.The key point of prevention and treatment is thorough root canal preparation and normative manipulation.
