胰岛素泵治疗初诊妊娠糖尿病的疗效分析

目的探讨胰岛素泵用于治疗初诊妊娠糖尿病的临床疗效与价值。方法选择2009年3月-2013年6月我科收治的50例妊娠期糖尿病患者，随机分为A、B两组，每组各25例。A组使用连续皮下注射胰岛素进行治疗，B组采用胰岛素泵进行治疗。比较两组患者治疗前后血糖变化及孕妇、围产儿并发症发生率等指标。结果治疗前，B组血糖值与A组无明显差异（P〉0．05），治疗后，B组血糖值显著优于A组（P〈0．05）；B组使用药物剂量明显少于A组（P〈0.05），且B组糖化血红蛋白值指标优于A组（P〈0．05）。B组母体及新生儿发病率均显著低于A组（P〈0．05）。结论妊娠期糖尿病患者使用胰岛素泵对血糖进行控制疗效明显，能够使空腹血糖及餐后血糖尽量接近正常值，尽可能降低患者低血糖发生率，保证孕妇及胎儿安全．值得临床推广应用。

Effects on insulin pump therapy in newly diagnosed with gestational diabetes mellitus

Objective To investigate the clinical effect and value of insulin pump used in treatment of patients with gestational diabetes mellitus. Methods selected 50 cases of gestational diabetes from March 2009 to June 2013 in our hospital,and the cases were randomly divided into group A and B, 25 patients in each group. Group A used continuous subcutaneous insulin injection, group B were treated with insulin pump. Two groups were compared before and after treatment in blood glucose change and pregnant women, perinatal complications were. Results Before treatment, blood glucose value in group B had no obvious difference with group A （P〉0.05）, after treatment, blood glucose value in group B was significantly better than group A （P〈0.05）; dosage in group B was less than group A （P〈0.05）, and glycosylated hemoglobin value index in group B was better than group A （P 〈0.05）. Maternal and neonatal morbidity in group B were significantly lower than in group A （P〈0.05）. Conclusion Control effect of gestational diabetes patients with insulin pump on blood sugar is obviously, can make the fasting blood glucose and postprandialblood glucose levels as close to normal value, reduce the incidence ofhypoglycemia in patients with as much as possible, to ensure the safety of pregnant women and the fetus, worthy of clinical application.
