两种术式治疗82例肝包虫的疗效对比分析

目的：对临床两种术式治疗肝包虫的疗效进行分析评价，为手术术式的选择提供参考。方法：收集我科4年来两种手术方式治疗的82例患者，比较手术耗时、术中出血、住院时间、带管时间、残腔感染、胆漏、原住复发等，并对结果进行统计学分析。结果：外膜内完整外囊摘除术与内囊摘除术＋外囊残腔引流术相比，两组手术耗时、住院时间、带管时间比较差异有统计学意义（P〈0．05），两组术中出血之间比较差异无统计学意义（P〉0．05）。两组并发症比较，残腔感染，原位复发的差异有统计学意义（P〈0．05），两组胆漏之间比较差异无统计学意义（P〉0．05）。结论：根治性肝包虫病外科治疗术式中，外膜内完整外囊摘除术具有创伤小、带管时间短、并发症少等优点。

Comparative analysis of efficacy on two kinds of surgical procedure for hepatic hydatid in 82 cases

Objectives： To evaluate and compare the clinical efficacy of two kinds of surgical procedure for hepatic hydatid. Methods： Data of 82 patients treated by two kinds of operative procedures were analysed and compared during four years in our hospitai, including time consuming during operation , the amount of bleeding ,the hospital stay ,time for drainage, residual cavity infection , biliary fistula , in-situ recurrence etc. Results： The integrity of the membrane sac excision comparing with excision of internal sac, the time consuming during operation , hospitalization days , the time for drainage had significant difference （P〈0.05）, There were no statistical different in the amount of bleeding（P〉0.05 ）. There were statistical different in complication residual cavity infection, in-situ recurrence（P〈0.05 ）, There were no statistical different in biliary fistula ,（P〉0.05 ）. Conclusion： In the radical surgical treatment of liver hydatid disease , the integrity of the capsular membrane extraction have advantages：trauma slightly,time for drainage shortly,complication less.
