肿瘤标志物联合检测对常见恶性肿瘤的筛查作用

目的 探讨甲胎蛋白（AFP）、癌胚抗原（CEA）、糖类抗原199（CA199）联合检测对常见恶性肿瘤的筛查作用。方法 应用电化学发光法联合检测AFP、CEA、CA199在各种常见恶性肿瘤中的含量，计算并比较其相应的敏感性和特异性。结果 肝癌患者AFP及CA199敏感性明显优于CEA；肺癌及大肠癌患者CEA及CA199明显优于AFP；CA199在各种常见恶性肿瘤中敏感性均较高。联合检测3种肿瘤标志物在常见肿瘤中可出现一项或一项以上的阳性结果，与其他良性疾病组及健康对照组比较，差异有统计学意义。结论 联合检测3种肿瘤标志物有助于提高常见恶性肿瘤的早期检出率。

Screening of combined detection of tumor markers for common malignant tumors

Objective To explore the sensitivity and specificity of combined detection of serum AFP,CEA and CA199 in common malignant tumors. Methods Serum levels of AFP, CEA and CA199 were measured with chemiluminescent immunoassay （CLIA） in patients with common malignant tumors or benign diseases and healthy adults. The sensitivity and specificity of the serum markers were evaluated and compared. Results The sensitivity of AFP and CA199 was higher than that of CEA in patients with hepatocarcinoma, while the sensitivity of CEA and CA199 were higher than that of AFP in patients with lung cancer and carcinoma of large intestine. The sensitivity of CA199 was higher in patients with common malignant tumors. Compared with that of benign diseases group and healthy control group, the positive result of one or more serum markers in patients with common malignant tumors was more frequent than that of combined detection of three tumor markers. The difference was statistically significant. Conclusion Combined detection of three serum tumor markers （AFP, CEA and CA199） contributes to the improvement of early detection rate of common malignant tumors.
