极速脉搏波传导速度技术在冠心病心绞痛患者颈动脉粥样硬化风险评估中的应用

摘    要:目的 探讨超声极速脉搏波技术UFPWV在评估冠心病心绞痛患者颈动脉粥样硬化中的应用价值。方法 选择2020年6月至2021年6月本院收治的冠心病心绞痛患者64例为研究组,按照心绞痛的类型分为稳定组(稳定型心绞痛)34例和不稳定组(不稳定型心绞痛)30例;选取同期的健康志愿者30例为对照组。收集患者的一般资料,二维灰阶超声记录颈动脉内中膜厚度、左心室射血分数LVEF;采用极速超声彩色脉搏波技术检测收缩早期脉搏波速度(PWVBS)、收缩晚期脉搏波速度(PWVES);动脉硬化指标采用酶联免疫吸附法检测血清基质金属蛋白酶-3(MMP-3)、蛋白酶K水平。结果 与对照组相比,研究组患者吸烟、收缩压、颈动脉内膜、PWV-BS、PWV-ES均高于对照组,差异有统计学意义(P<0.05)。与稳定组比较,不稳定组颈动脉PWV-ES高于稳定组,差异有统计学意义(P<0.05)。与对照组相比,稳定组和不稳定组患者血清MMP-3、蛋白酶K明显高于对照组,与稳定组相比,不稳定组血清MMP-3、蛋白酶K明显高于稳定组,差异均有统计学意义(P均<0.05)。PWV-ES高水平患者动脉硬化指标明显高于PWV-ES低水平者,差异均有统计学意义(P均<0.05)。结论UFPWV技术能够评估冠心病心绞痛患者颈动脉壁弹性变化,具有一定的临床应用价值。

Abstract:Objective To explore the value of ultra fast pulse wave (ufpwv) in evaluating carotid atherosclerosis in patients with coronary heart disease and angina pectoris.Method 64 patients with coronary heart disease and angina pectoris treated in our hospital from June 2020 to June 2021 were selected as the study group. According to the type of angina pectoris, they were divided into 34 cases in the stable group (stable angina pectoris) and 30 cases in the unstable group (unstable angina pectoris); 30 healthy volunteers in the same period were selected as the control group. The general data of patients were collected. Carotid intimal thickness and left ventricular ejection fraction LVEF were recorded by two-dimensional gray-scale ultrasound; The pulse wave velocity in early systole (pwvbs) and late systole (pwves) were detected by rapid ultrasound color pulse wave technique; The levels of arteriosclerosis index metalloproteinase-3 (MMP-3) and protease K were detected by enzyme-linked immunosorbent assay.Result Compared with the control group, smoking, systolic blood pressure, carotid intima, pwv-bs and pwv-es in the study group were significantly higher than those in the control group (P < 0.05).Compared with the stable group, the carotid pwv-es in the unstable group was significantly higher than that in the stable group (P < 0.05). Compared with the control group, the serum MMP-3 and protease K in the stable group and unstable group were significantly higher than those in the control group. Compared with the stable group, the serum MMP-3 and protease K in the unstable group were significantly higher than those in the stable group (all P < 0.05).The index of arteriosclerosis index in patients with high level of pwv-es was significantly higher than that in patients with low level of pwv-es (all P < 0.05). Conclusion Ufpwv can evaluate the changes of carotid wall elasticity in patients with coronary heart disease and angina pectoris, and has certain clinical value.[Key words] Pulse wave technology; Coronary heart disease; Angina pectoris; Carotid atherosclerosis
