小儿免气腹单孔腹腔镜阑尾切除术与经脐单孔腹腔镜阑尾切除术的对比研究

目的:探讨免气腹单孔腹腔镜阑尾切除术的手术效果及方法。方法:将180例阑尾炎患儿随机分为两组,100例行免气腹单孔腹腔镜阑尾切除术(A组),80例行经脐单孔腹腔镜阑尾切除术(B组),对比两组手术时间、术后下床活动时间、术后胃肠功能恢复时间、术后住院时间及住院费用等指标。结果:180例手术均获成功。两组手术时间、住院费用差异无统计学意义(P>0.05),A组术后下床活动时间、术后胃肠功能恢复时间、术后住院时间显著短于B组(P<0.001)。结论:免气腹单孔腹腔镜阑尾切除术具有创伤更小、康复快、住院时间短、不增加费用等优点,是安全、有效的,在基层医院可推广应用。

A comparative study of gasless one-port laparoscopic appendectomy and single-port transumbilical laparoscopic appendectomy for child

Objective： To explore the effect and method of gasless one-port laparoscopic appendectomy. Methods： One hundred and eighty children with appendicitis were randomly divided into two groups： group A underwent gasless one-port laparoscopic appendectomy（ n = 100）,group B was performed single-port transumbilical laparoscopic appendectomy（ n = 80）. Operation time,postoperative out-of-bed activity time,gastrointestinal function recovery time,postoperative hospital stay,hospitalization expenses and other indicators of two groups were compared. Results： All 180 cases of appendicitis operation were successful,operation time（ P = 0. 221） and hospitalization expenses（ P = 0. 186） of two groups were similar. Postoperative out-of-bed activity time,gastrointestinal function recovery time,hospital stay of group A were significantly shorter than those of group B（ P = 0. 000）. Conclusions： Gasless one-port laparoscopic appendectomy is safe and effective,has the advantages of less trauma,quicker recovery,shorter hospitalization time,without cost increasing. It could be popularized in primary hospital.
