新诊断食管癌患者胰岛素抵抗状态分析

目的观察食管癌患者胰岛素抵抗状态,为肿瘤防治提供指导。方法测定150例新诊断食管癌和68例健康对照者的空腹胰岛素(FINS)和C肽水平(FC-P),并用稳态模式评估法(HOMA)计算胰岛素抵抗指数。结果 (1)早发组和晚发组FINS明显高于对照组(P0.05),早发组和晚发组HOMA-IR明显高于对照组(P0.01);(2)早发组FINS和HOMA-IR均高于晚发组(P0.05)。但两指标与肿瘤分期无关。结论胰岛素抵抗是食管癌早期发病机制之一,提示对小于50岁,存在胰岛素抵抗人群加强食管癌筛查有重要的意义。

Newly diagnosed insulin resistance in patients with esophageal cancer

Objective To observe the state of insulin resistance in patients with esophageal cancer for tumor prevention and treatment guidance.Methods 150 patients with newly diagnosed esophageal cancer and 68 healthy controls were divided into early-onset group,late-onset group,and the control group.Fasting insulin and C peptide levels and homeostasis model assessment（HOMA） method were used for index of insulin resistance.Results In early-onset group and late-onset group FINS was significantly higher（P0.05）,and in early-onset group and late-onset group HOMA-IR was significantly higher（P0.01）.In early-onset group FINS and HOMA-IR were higher than those in late-onset group（P0.05）.However,the two indicators had nothing to do with the tumor stages（P0.05）.Conclusion Insulin resistance is one of the pathogenesis of early esophageal cancer,suggesting for people with less than 50 years of age who have the existence of insulin resistance to strengthen screening of esophageal cancer has important significance.
