双源CT对冠状动脉钙化斑块与冠状动脉狭窄程度关系分析

摘    要:目的研究冠状动脉钙化斑块(coronary artery calcified plaque,CACP)的多种参数和冠状动脉血管狭窄程度间的关系,探讨使用CACP参数评估冠状动脉狭窄程度的可行性。方法回顾性分析自2010年2月—2010年6月70例临床疑似冠心病人双源CT扫描资料,包括冠状动脉CT动脉造影(coronary CT angiography,CCTA)资料和CACP资料。使用多样本方差及多元回归统计分析,分别比较所有病例冠状动脉左回旋支(LCX)、左主干(LM)、左前降支(LAD)、右冠状动脉(RCA)以及总体CACP有关参数与相对应血管狭窄程度的关系。结果无冠状动脉狭窄组与冠状动脉狭窄组之间以及冠状动脉不同等级狭窄组之间CACP各项参数上有显著性差异,差异结果受CACP参数类型和血管分支类型的影响。可以建立CACP多种参数与血管狭窄程度的多元回归方程。提示冠状动脉狭窄程度的一个重要影响参数是CACP数目。结论对CACP的分析应考虑到参数类型和血管分支类型不同所导致的差异性。应提高对CACP数目这一参数的认识,CACP数目越多提示冠状动脉狭窄程度越严重。可以通过建立的多元回归方程使用CACP参数评估冠状动脉血管的狭窄程度。

Relationship of coronary artery calcification plaques and coronary artery stenosis degree analyzed by Dual-source CT

Abstract:Objective To explore the relationship between various calcified plaque evaluation parameters and the degree of coronary artery stenosis, and to study the feasibility of evaluating the degree of coronary artery stenosis with CACP. Methods Retrospective analysis was made of 70 suspected coronary heart disease patients. Dual-source CT coronary artery (CTA) images from February 2010 to June 2010 and related CACP parameters were studied. The relationship wad compared between all LCX, LM, LAD, RCA as well as total CACP related parameters with the related degree of hemal stricture. Results There were obvious CACP differences in various parameters through comparison of patients without coronary artery stenosis and patients of coronary artery stenosis with different degrees of the narrow group. The differences were influenced by types of CACP parameters and type of vascular branch. General multivariate regression equation was established of corresponding CACP various parameters and vascular stenosis. Multiple regression analysis showed that the number of CACP was a major influence that parameter for the degree of coronary artery stenosis. Conclusion The differences resulted from types of CACP parameters and type of vascular branch should be consider in the analysis of CACP. We should pay more attention to numbers of CACP. The more numbers of CACP, the more serious the degree of coronary artery stenosis. We can use parameters of CACP to evaluate the degree of coronary artery stenosis through establishing multivariate regression equation.
