不同介入疗法对胃癌肝转移瘤疗效的分析

目的探讨经肝动脉灌注化疗栓塞术和单纯肝动脉灌注术治疗胃癌肝转移瘤疗效。方法分析57例胃癌肝转移瘤DSA表现,介入治疗方案选择和效果评价。结果 57例中,27例为富血供者,乏血供者30例,富血供者除2例弥漫性病灶外,25例富血供者行经肝动脉灌注化疗栓塞术,余患者32例行单纯肝动脉灌注化疗术。与单纯肝动脉灌注化疗组相比,肝动脉灌注化疗栓塞术治疗胃癌肝转移在肿瘤有效率和控制率有明显提高,TACE组中位生存期为16个月,TAI组为11个月,两组间存在也存在差异(P0.05)。结论对于富血供胃癌肝转移瘤,肝动脉化疗栓塞治疗是安全有效的治疗方法,与单纯经肝动脉灌化疗相比能明显提高疗效。

Different interventional treatment of liver metastasis in the patients with gastric cancer

Objective To investigate the efficacy of transcatheter arterial chemoembolization(TACE) and transcatheter arterial infusion(TAI) in the patient with liver metastasis from gastric cancer.Methods DSA images findings for 57 patients with the liver metastasises from gastric cancer were analysed,and selection of intervention therapy and therapeutic efficacy were evaluated.Results Among 57 patients,27 had abundant blood supply,and 30 had poor blood supply.All patients with hypervascular blood supply were treated by TACE except the 2 patients with excessive focus,32 patients were treated by TAI.Compared with that of TAI alone,tumour therapeutic response rate and control rate in those under going TACE was statistically higher.The median survival time of the TACE group was 16 months,and 11 months in the TAI group(P<0.05).Conclusion TACE is an important and safe treatment of the liver metastasis from gastric cancer,the efficacy of which is better than TAI.
