12例妊娠中晚期合并急性梗阻性化脓性胆管炎治疗的探讨

目的探讨妊娠中晚期伴急性梗阻性化脓性胆管炎的临床特点和治疗经验。方法对2005～2010年间该院12例妊振中晚期伴急性梗阻性化脓性胆管炎的患者治疗效果进行回顾分析。结果 2例中期妊娠患者行EST手术,待预产期剖宫产,母婴平安。10例晚期妊娠患者行胆囊切除+胆总管切开探查取石+T管外引流术+剖宫产终止妊娠术,1例母婴死亡,9例母婴痊愈或好转出院。结论妊娠中晚期伴发急性梗阻性化脓性胆管炎是一类凶险度极高疾病,一旦确诊,应及时手术治疗同时终止妊娠,对母婴健康颇有裨益。

Treatment of 12 cases of pregnancy patients in intermediate and advanced stage with acute obstructive suppurative cholagitis

Objective To study the clinical feature and therapy method of pregnancy patients with acute obstructive suppurative cholagitis （AOSC）. Methods Twelve cases of pregnancy patients in intermediate and advanced stage with acute obstructive suppurative cholagitis were analyzed retrospectively from 2005 to 2010. Results Two cases of pregnancy patients in intermediate stage were subject to EST, who were safe to expected date of delivery and were successfully done with abdominal delivery. 10 cases of pregnancy patients in ad- vanced stage were subject to gallbladder resection,bile duct resection, ＂T＂ drainage and termination of pregnancy operation, who recov- ered well except for one case dead. Conclusion Pregnancy with acute obstructive suppurative cholagitis is a dangerous disease to deal with. The point for pregnancy women and fetus is prompt operation and termination of pregnancy.
