妊娠晚期妇女甲状腺功能变化Meta分析

目的通过Meta分析研究妊娠晚期妇女甲状腺功能变化特点。方法收集国内外多个数据库符合入选标准的队列研究,采用随机效应模型对妊娠晚期妇女促甲状腺激素(TSH)和游离甲状腺激素(FT4)水平进行Meta分析。结果纳入符合标准的7项研究,采用随机效应模型进行汇总分析,TSH均数之差的合并效应值为0.09,95%CI分别为-1.32、1.5,提示妊娠晚期组与非妊娠对照组TSH水平差异无显著性(P0.05);纳入符合标准的8项研究,采用随机效应模型进行汇总分析,其结果示FT4均数之差的合并效应值为-3.23,95%CI分别为-4.23、-2.23,提示妊娠晚期组较非妊娠对照组FT4降低,差异有显著性(P0.01)。结论妊娠晚期妇女的FT4水平较非妊娠对照组显著降低,而未见TSH水平相应升高,TSH变化特点有待进一步探究。

A meta-analysis on thyroid function of women during late trimester of pregnancy

Objective To explore change of thyroid function of women during late trimester of pregnancy by meta analysis.Methods We collected relevant cohort studies from many database,data of TSH,FT4 of all objects were analyzed by random effect model.Results Seven cohort studies were collected,random effect model was applied to analyze the data of TSH.Weighted mean difference of TSH was 0.09,95% confidence interval was-1.32,1.5.The results indicated that there no diference between pregnant women and the control group,however it was not statistically significant.Eight cohort studies was collected,random effect model was applied to analyze the data of FT4.Weighted mean difference of FT4 was-3.23,95% confidence interval was-4.23、-2.23.The results indicated that the level of FT4 of women during late trimester of pregnancy was significantly lower than that of the control group（P0.01）.Conclusion The FT4 level of women during late trimester of pregnancy is significantly lower than that of the control group,however level of TSH doesn＇t rise accordingly,which needs more further research.
