小儿消化性溃疡并幽门梗阻17例临床分析

目的探讨和总结小儿消化性溃疡致幽门梗阻的临床特点和治疗方法。方法对我院1996年9月-2004年9月收治的17例小儿消化性溃疡致幽门梗阻患儿的病例资料进行回顾性分析、评价。结果17例患儿均行幽门成型术，全部治愈出院，随诊3个月～5年，术后有2例患儿经胃镜检查发现胃窦部浅表性溃疡，内科治疗后痊愈，全组患儿生长发育正常。结论小儿消化性溃疡致幽门梗阻的病史及临床表现不典型，其诊断应结合纤维胃镜及影像学的检查，确诊后采用幽门成型术，疗效肯定，术后并发症少。

Clinical Analysis of 17 Cases of Pyloric Obstruction Complicated With Infantile Peptic Ulcet

Objective To explore and summarize clinical feature and therapy of the pyloric obstruction caused by infantile peptic ulcer. Methods Clinical data of 17 cases of pyloric obstruction complicated with infantile peptic ulcer were analyzed and evaluated retrospectively,these sick children were hospitalization during Sep. 1996 to Sep 2004 in our hospital. 17 cases of pyloric obstruction underwent pyloroplasty add medical trestment,all patients had recovery well,simultaneously they were followed for 3 months to 5 years after discharge. Results These finding showed that there were 2 cases of superficial ulcer in gastric sinus,we cured the younger children with superficial ulcer by using medical treatment their growth and development were all normal. Conclusions Because disease history and clinical manifestation of pyloric obstruction induced by infantile peptic ulcer-induced were no tipical. Therefore, its diagnosis have to combine fibrogastroscopy with imaging examination. Once the disease diagnosis was defined,using pyloroplasty with medical treatment could obtain a confirmed efficacy immediately,and less complication in postoperation.
