老年外伤性迟发性脑内血肿60例临床分析

目的　探讨老年外伤性迟发性脑内血肿的原因、特点、诊断和治疗。方法　回顾性分析我院自 1999～ 2 0 0 3年收治 60例老年外伤性迟发性脑内血肿。结果　 60例中行血肿清除术 3 0例 ,钻孔血肿腔引流 9例 ,非手术治疗 2 1例。按格拉斯哥预后评分(GOS) :良好 10例 ,轻残 14例 ,重伤 10例 ,植物生存状态 8例 ,死亡 18例。结论　进行性意识障碍、头痛、呕吐 ,生命体征的异常波动是本病的特征 ,及时复查CT扫描有利于早期诊断 ,掌握手术及非手术治疗的指征 ,是治疗本病的关键。

The clinical analysis of elderly traumatic delayed intracranial hematoma

Objective To explore the reason, feature, diagnosis and treatment of the elderly traumatic intracranial hematoma. Methods Reviews and analysis were made of the 60 cases of elderly traumatic delayed intracranial hematoma who were hospitalized for treatment between 1999 and 2003. Results There were 30 cases of hematoma centesis operation, 9 cases of drilled hematoma drainage, 21 cases of non-operative treatment. GOS ratings: 10 patients relieved, 14 in lowgrade deformity, 10 in severeness, 8 in persistent vegetative status and 18 died. Reason for Death: 12 cases died of syndrome and 18 died of idiopathy brain damage.Conclusion The abnormal fluctuation of progressing consciousness obstruction, headache, vomit and vital signs are the disease's features. Timely reviews of CT scanning is in favor of early diagnosis. Grasping the indication of operative and non-operative treatments is the key of treating the disease. Incidence rate of syndrome is high.
