二肽基肽酶Ⅳ与卵巢上皮性癌临床病理特征和预后的关系

目的 探讨卵巢上皮性癌组织中二肽基肽酶N(DPPIV)的表达及临床意义.方法 采用免疫组化法榆测378例卵巢上皮性癌组织中DPPIC蛋白的表达,采用原位杂交法检测86例卵巢上皮性癌组织中DPPIV mRNA的表达,42例正常卵巢组织作为对照.结果 DPPIV蛋白在卵巢上皮件癌组织中的总阳性表达率为92.9%(351/378),在正常卵巢组织中的总阳性表达率为59.5%(25/42),差异有统计学意义(P=0.001).DPPIV蛋门的表达与患者的发病年龄、肿瘤分化程度和FIGO分期无关(均P＞0.05),而与肿瘤的组织学类型有关(P=0.005).DPPIC蛋白的表达水平越高,患者的生存期越短(P=0.023).DPPIV mRNA在卵巢上皮性癌组织巾的总阳性表达率为97.7%(84/86),在正常卵巢组织中的总阳件表达率为64.3%(27/42),差异有统计学意义(P＜0.05).卵巢上皮性癌组织中DPPIV蛋白和mRNA的表达与定位一致,且二者旱止相关(P=0.001).结论 DPPIV在卵巢上皮性癌的发生发展中起着一定作用,其表达水平可能成为影响患者预后的因素.

Correlation of DPP IV expression with clinicopathological features and prognosis in epithelial ovarian carcinoma

Objective To investigate the expression of dipeptidyl peptidase IV(DPPN)in patients with epithelial ovarian carcinoma(EOC)and its clinical significance.Methods Immunolhistochemistry(IHC)was used to detect the expression of DPP IV protein in 378 formalin-fixed paraffin-embedded EOC tissue samples.The expression of DPPIV mRNA in 86 EOC tissue samples were examined by in situ hybridization (ISH)using specific FITC-labelled RNA probes.Forty-two samples of normal ovarian tissues were used as control.Statistical analyses were carried out by Chi-square test,Spearman rank correlation and Kaplan-Meier method.Results Among the 378 epithelial ovarian carcinomas,351(92.9%)showed a positive expression of DPP IV protein.while only 25/42(59.5%)of normaI ovaries had a positive expression by semiquantitative IHC analysis.The expression level of DPP IV protein was significantly lower in the normal ovaries than that in ovarian carcinomas(X2=18.4,P=0.001).There was no significant correlation between the expression of DPP IV protein and age,FIGO stage and histological grade(P＞0.05).However,the expression of DPPIV protein was significantly associated with histological type(X2=28.5,P=0.005).The patients with high level expression of DPP IV protein likely had a poor prognosis in terms of overall survival (P=0.02).Of the 86 patients,84(97.7%)showed positive expression of DPPN mRNA,also higber than that in normal ovarian tissues(P＜0.05). A statistically significant correlation between DPPIV mRNA and protein expression was observed(r=0.66,P=0.001).Conclusion DPI IV may be involved in the carcinogenesis of ovarian cancer,and may become a potential prognostic marker for epithelial ovarian carcinoma.
