CT对卵巢原发性恶性肿瘤的诊断价值

目的 提高CT对卵巢原发性恶性肿瘤的诊断正确性。方法 回顾性分析49例80个经手术和病理证实有完整资料的卵巢原发性恶性肿瘤的CT表现特点。结果 49例80个肿瘤中75个瘤体边缘不规则或呈分叶状，5个轮廓较光整。肿瘤以囊性为主者14个，混合性35个，以实性为主者31个。11例瘤体内见粒状、斑片状或环状钙化。7例可见扭曲增粗的肿瘤血管。除2例瘤体较小者外，47例均可见周围脏器受压移位或直接受侵征象。腹水11例，胸腔积液6例，17例腹腔种植，1例肝转移。结论 卵巢原发性恶性肿瘤的CT主要表现为囊实性肿块，CT可对卵巢肿瘤准确定位并作出良恶性定性诊断。

Utility of CT in the diagnosis of primary ovarian carcinoma

Objective To improve the CT diagnostic accuracy in primary ovarian carcinoma.Methods The CT characteristics of 49 patients with surgically and pathologically proved primary ovarian carcinoma were analyzed retrospectively.Results Among 80 tumors in 49 cases,75 tumors were irregular borders or lobulated while remaining 5 tumors were smooth and complete contour.All tumors were classified into three patterns,i.e. predominantly cystic pattern(more than two-third of the mass is cystic,n=14),mixed pattern (one-third to two-third of the mass is solid,n=35) and solid pattern(more than two-third of the mass is solid,n=31).The intratumoral granular,patch or circular calcifications were found in 11 cases.The flexuous and thickned tumoral vessels were found in 7 cases.The oppression and displacement or direct invasion of neighbor organs were found in 47 cases,except 2 cases with less tumors.Ascites (n=11),pleural effusion(n=6),abbdominal cavity implants (n=17)and liver metastasis (n=1) were also found in the group patients.Conclusion CT characteristics of primary ovarian carcinoma is main cystic and solid masses.CT imaging can not only determine the location but differentiate benign or malignant ovarian tumors.
