SH2B1基因多态性与妊娠糖尿病的相关性研究

摘    要:探讨SH2 B1基因多态性与妊娠糖尿病患者胰岛素抵抗的关系。方法选取164 例妊娠期糖尿病(GDM)患者和130 例糖耐量正常孕妇。采用限制性片段多态性聚合酶链反应检测SH2B 1 基因型,并且分析相关临床资料。结果GDM 组SH2B 1(rs4788102)基因 GA型与 GG 型比较,差异有统计学意义OR=1.531,(95%CI:1.093,2.374) =0.04],A等位基因频率与G 等位基因频率比较,差异有统计学意义OR=1.436,(95%CI:1.091,1.972) =0.01]。GDM 组 + 基因型与 基因型比较,差异有统计学意义 OR=1.699,(95%CI:1.185,2.479) =0.02]。GDM 组中+ 基因型体重指数、三酰甘油、瘦素及稳态模型评估胰岛素抵抗指数与基因型比较,差异有统计学意义( p<0.05)。经Logistic回归分析显示,GA +AA 基因型为稳态模型法测定胰岛素抵抗指数升高的危险因素。结论SH2B 1(rs4788102)基因多态性可能是GDM的一个易感位点,监测孕妇该基因位点,可以用于GDM 发生风险的预测。

Abstract:To investigate the relationship between gene polymorphism and insulin resis-tance in patients with gestational diabetes mellitus (GDM). Methods Totally 164 GDM patients and 130 normal pregnant women with impaired glucose tolerance were selected. The genotype was detected by poly-merase chain reaction-restriction fragment length polymorphism and then the clinical data were analyzed. Results There was significant difference between the GA and GG genotypes of (rs4788102) in the GDM group OR = 1.531, (95% CI: 1.093, 2.374), = 0.04]. There was significant difference in the A allele frequency and the G allele frequency in the GDM group OR = 1.436, (95% CI: 1.091, 1.972), = 0.01]. In the GDM group, the GA and AA genotypes were significantly different from the GG genotype OR = 1.699,(95% CI: 1.185, 2.479), = 0.02]. BMI, TG, leptin, and HOMA-IR were significantly different between the GDM patients with AA or GA genotype and those with GG genotype ( P< 0.05). Logistic regression analysis showed that GA and AA genotypes were the risk factors for HOMA -IR elevation. Conclusions (rs4788102) gene polymorphism may be a susceptible locus of GDM, which could be used to predict the risk of GDM in pregnant women.
