一氧化氮吸入联合高频通气对呼吸窘迫综合征患儿血气状态及并发症的影响

摘    要:目的 探讨吸入一氧化氮(NO)联合高频振荡通气(HFOV)方案用于重度呼吸窘迫综合征(RDS)患 儿的疗效及安全性。方法 选取2016年2月—2019年2月于辽宁省健康产业集团阜新矿总医院收治的62例重度 RDS患儿。按治疗方式分为两组,30例接受常频通气+NO吸入治疗患儿作为对照组,32例接受HFOV+NO吸 入治疗患儿作为研究组,分析两组患儿血气状态改善、并发症等情况。结果 研究组呼吸机治疗时间较对照组长 (P <0.05)。研究组治疗后的pH、SaO2、PaO2、OI较对照组高(P <0.05),PaCO2、肺泡-动脉血氧分压差较对照 组低(P <0.05)。研究组治疗后OI较对照组低(P <0.05)。两组并发症发生率比较,差异无统计学意义(P >0.05)。 结论 HFOV+NO可有效改善RDS患儿血气状态,并发症较少,获得良好的疗效及安全性。

Abstract:Objective To evaluate the efficacy and safety of NO inhalation combined with high frequency oscillatory ventilation (HFOV) in the treatment of neonates with respiratory distress syndrome (RDS). Methods Sixty-two children with RDS admitted to our hospital from February 2016 to February 2019 were divided into two groups according to the treatment methods, with thirty children received CMV + NO inhalation as the control group and thirty-two children received HFOV+NO inhalation as study group. The improvement of blood gas status and the occurrence of complications were retrospectively analyzed. Results The treatment time of the study group was longer than that of the control group (P < 0.05); After 4, 12 and 24 hours, PaCO2 and A-A PO2 in the Study Group were lower than those in the control group (P < 0.05); PH, PaO2, OI, and SaO2 were higher than those in the control group (P < 0.05); compared with the control group, the value of OI at 4, 12 and 24 hours after treatment in the study group was lower (P < 0.05); the total incidence of complications in the study group was 15.63%, compared with 10.00% in the control group, but there was no significant difference (P > 0.05). Conclusion NO inhalation combined with high frequency ventilation used in the treatment of children with RDS can effectively improve the blood gas state with few complications and obtain good therapeutic effectiveness and safety.
