急性脑梗死患者血清IMA、LP-PLA2 水平及与病情程度的相关性

摘    要:目的 探究急性脑梗死(ACI)患者血清缺血修饰白蛋白(IMA)、脂蛋白相关磷脂酶A2(LPPLA2)水平变化,并分析患者血清IMA、LP-PLA2 水平与病情的相关性。方法 选取2017 年6 月—2018 年12 月在梧州市红十字会医院确诊的ACI 患者122 例作为观察组。根据脑梗死面积将观察组122 例分为轻度梗死组35 例、中度梗死组57 例和重度梗死组30 例。另选取同期该院健康体检者62 例作为对照组。分别检测各组血清IMA、LP-PLA2 水平。采用Pearson 相关分析观察组血清IMA、LP-PLA2 水平与病情的相关性。结果 观察组血清IMA、LP-PLA2 水平高于对照组(P <0.05)。中重度梗死组、中度梗死组血清IMA、LPPLA2水平高于轻度梗死组(P <0.05),且重度梗死组高于中度梗死组(P <0.05)。Pearson 相关分析显示,患者血清IMA 水平与病情(NIHSS 分数)呈正相关(r =0.738,P =0.000)。患者血清LP-PLA2 水平与病情(NIHSS分数)呈正相关(r =0.671,P =0.000)。结论 ACI 患者血清IMA、LP-PLA2 水平升高。随着梗死面积增大,其血清IMA、LP-PLA2 水平越高,且呈正相关。血清IMA、LP-PLA2 水平可以作为判断ACI 病情的标志物。

Abstract:Objective To investigate the changes of serum ischemia-modified albumin (IMA) and lipoproteinassociatedphospholipase A2 (LP-PLA2) levels in patients with acute cerebral infarction (ACI), and to analyze thecorrelation between the levels of both IMA and LP-PLA2 and the disease. Methods Totaly 122 patients with ACIdiagnosed in our hospital were selected as the observation group from June 2017 to December 2018. According to theinfarct area, 122 patients in the observation group were divided into three subgroups: minor infarct group (n = 35),medium infarct group (n = 57) and severe infarct group (n = 30). 62 healthy persons were selected as the control groupin the same period. The serum levels of IMA and LP-PLA2 were detected. Pearson correlation analysis was used toobserve the correlation between serum IMA, LP-PLA2 and the disease. Results The levels of serum IMA and LPPLA2in the observation group were higher than those in the control group (P < 0.05). The levels of serum IMA andLP-PLA2 in the severe and medium group were higher than those in the minor group (P < 0.05). The levels of serumIMA and LP-PLA2 in severe group was higher than that in medium group (P < 0.05). Pearson correlation analysisshowed that there was a positive correlation between serum IMA level and NIHSS score (r = 0.738, P = 0.000). Therewas a positive correlation between serum LP-PLA2 level and NIHSS score (r = 0.671, P = 0.000). Conclusion The levels of serum IMA and LP-PLA2 in ACI patients increase. The levels of serum IMA and LP-PLA2 were higher andpositively correlated with the increase of infarct area. The serum levels of IMA and LP-PLA2 can be used as markersto judge the condition of ACI.
