机械辅助排痰应用于老年慢性阻塞性肺疾病急性加重期患者的临床观察

目的评价机械辅助排痰机在老年慢性阻塞性肺疾病急性加重期（AECOPD）患者的应用效果。方法43例AECOPD患者在常规抗感染和对症治疗基础上随机分为。实验组21例和对照组22例，实验组采用机械辅助加体位引流排痰，对照组采用人工扣击法加体位引流排痰。4d后观察两组患者心率、呼吸及血气指标变化，比较两组排痰效果。结果实验组咳嗽、痰量、肺部罗音、呼吸困难等临床症状变化有效率与对照组相比有显著性（P〈0．05）。血气分析结果显示实验组治疗前后比较差异有显著性（P〈0．05）。结论机械辅助排痰效果优于传统的人工扣击排痰。

Clinical applying mechanical assistance for expulsion of phlegm of patients with AECOPD

Objective To evaluate effects of enhancing expulsion of phlegm with mechanical assistance on old patients with AECOPD. Methods Based on routine treatment against infections and symptom-specific therapy, 43 patients with AECOPD were randomly divided into two groups, including experiment group （21 cases） and control group （22 cases）. In the experiment group, expulsion of phlegm was conducted by mechanical assistance pulsing postural liquid-drawing, whereas manpower tapping technique pulsing postural liquid-drawing was applied in the control group. Then changes of the patient＇s parameters, such as heart rates, respiration and blood gas, were observed to make comparison of effect on expulsion of phlegm between these two groups 4 days later. Results The blood gas analyses results indicated significant differences between before and after treatment （P〈0. 05）. Conclusions The expulsion of phlegm by mechanical assistant was better than that by traditional manpower tapping technique.
