微创颅内血肿清除治疗基底节区脑出血疗效分析

摘    要:目的 观察在头颅CT引导下行微创颅内血肿清除术治疗基底节区脑出血的疗效.方法 选择CT证实基底节区脑出血患者120例,分为微创清除组(63例)和内科保守治疗组(57例),对两组患者治疗前后神经功能缺损评分、日常生活能力Barthel指数(BI)评分及血肿的清除情况进行比较.结果 微创组治疗前血肿体积为(48.66±9.09)ml,保守组为(48.31±7.06)ml.治疗7d后,微创组血肿体积为(12.38±4.35)ml,保守组为(25.40±3.23)ml,两组比较差异显著(P<0.01),63例血肿吸收明显快于57例保守组;治疗后,微创组神经功能缺损评分低于保守组,BI评分明显高于保守组,两者差异有统计学意义(P<0.05).结论 CT引导下颅内血肿微创清除术是治疗基底节区脑出血的一种积极有效的方法.

Abstract:Objective To investigate the efficacy of the minimally invasive drainage in treatment of the patients with cerebral hemorrhage under the head CT to guide.Methods 120 patients with cerebral hemorrhage confirmed by head CT scanning were randomly divided into mininmally invasive surgery group(n=63)and conservative treatment group(n=57).Before and after treatment,the neurological deficiency score,the ability of everyday activity and the hematoma obsorption rate in the 2 groups were compared.Results Before treatment,the volume of hematoma in minimally invasive group was(48.66±9.09)ml and the another group was(48.31±7.06)ml,after 7 days treatment,the volume of hematoma reduced to(12.38±4.35)ml and(25.40±3.23)ml respectively.After treatment,the neurological deficiency score in minimally invasive surgery group was better than those in the conservative treatment group and also in BI index.Conclusions The minimal invasive hematoma evacuation treatment is a effective method for basal ganglia cerebral hemorrhage.
